시ㆍ군ㆍ구 단위 지역사회 심뇌혈관 질환 표준화 사망률과 관련 요인 분석

국문 초록

본 연구는 시군구 통계지표를 이용하여 시군구 지역사회 특성에 따라 심뇌혈관 질환 표준화 사망률을 비교하고, 관련 요인을 분석하고자 수행하였다. 본 연구는 통계청의 국가통계포털 e-지방지표에서 인구수, 노인인구비율, 재정자립도, 의사수, 자동차등록대수를 이용하였으며, 국가통계포털 사망부분에서 사망원인 가운데 순환기계질환 표준화 사망률을 이용하였다. 또한 지역사회건강조사에서 시군구 단위의 건강행태자료를 이용하였다. 시군구 단위의 통계자료가 모두 제공되는 61개 시, 86개 군, 69개 구 지역을 분석의 대상으로 하였으며, SPSS 18.0을 이용하여 ANOVA, 상관관계 및 회귀분석을 실시하였다. 인구규모, 재정자립도, 인구 1000명당 의사수, 인구1인당 자동차 등록대수 등의 지역사회 특성에 따라 심뇌혈관 질환 사망률에 차이가 있는 것으로 나타났으며, 현재흡연율과 걷기실천율, 비만율 등이 심뇌혈관 질환 사망률과 상관관계가 있는 것으로 나타났다. 또한 심뇌혈관 질환 사망률에 영향을 주는 요인은 현재흡연율, 재정자립도, 인구 1인당 자동차 등록대수였으며, 이 회귀모형은 통계적으로 유의하여 적합한 것으로 나타났고, 이 세 변수에 의해 심뇌혈관 질환 사망률이 설명되는 변량은 35.8%였다. 지역사회 심뇌혈관 질환 사망률을 낮추기 위해서는 개인을 대상으로 하는 관리방법 외에 재정자립도 등 지역사회 간 자원배분에 대해서도 고려할 필요가 있다.

영문 초록

This study aims to compare cardiovascular disease-specific standardized mortality rate and explore the related factors in local districts using community indicators. Data were collected community health indicators of local districts from Statistics Korea and Community Health Survey 2010. Data of 216 local districts were used for the analysis. There were significant differences in cardiovascular disease-specific standardized mortality by size of population, financial independence, no. beds per 1000 population, no. of doctors per 1000 population, no. of registered car per capita. Prevalence of current smoking, walking over 30 min daily, and obesity were correlated with cardiovascular disease-specific standardized mortality. High current smoking rate, low financial independence, high no. of registered car per capita, high no. beds per 1000 population, low no. of doctors per 1000 population were significant predictors of cardiovascular disease mortality. Community health behaviors and other indicators could be influencing factors of community cardiovascular disease-specific mortality. Appropriate resource allocation need to be considered to reduce cardiovascular disease motality. Community health behaviors and other social indicators should be considered when making decisions on health policies and health planning.
