노인학대, 자아존중감, 일상생활수행능력(ADL) 및 정신사회적 요인과 노인 우울 간의 관련성

국문 초록

노인의 신체적, 정신적 요인과 노인학대에 의한 자아존중감이 노인우울에 영향을 미치는가를 규명하기 위해 서울과 경기도 일부 지역에 위치한 노인복지관 65세 이상 노인 1,000명을 대상으로 2010년 11월 8일부터 26일까지 3주에 걸쳐 설문조사를 실시하였다. 분석을 위한 모든 자료처리는 SPSS Ver. 12.0을 이용하였고, 구조방정식모형은 AMOS Ver. 7.0을 이용하여 분석하였다. 연구결과를 살펴보면, 첫째, 노인의 일상생활동작능력(ADL)은 우울에 정(+)의 영향을 미치는 것으로 나타났으며 (p＜0.01), 정신적 요인으로서 스트레스 또한 우울에 정(+)의 영향(p＜0.001)을 주는 것으로 분석되었다. 둘째, 노인학대는 자아존중감에 음(-)의 영향(p＜0.001)을 미치는 것으로 나타났으며, 자아존중감은 우울에 음(-)의 영향(p＜0.001)을 미치는 것으로 나타났다. 결론적으로 노인학대가 심할수록 우울의 정도가 증가하는 것으로 나타났다. 셋째, 사회적 지지가 정신적 요인인 스트레스에 음(-)의 영향을 주는 것으로 나타나(p＜0.001) 노인들의 사회적 지지가 높을수록 우울은 감소한다는 점을 알 수 있었다. 넷째, 가족간 갈등은 스트레스에 정(+)의 영향(p＜0.01)을 미치는 것으로 나타나 결국 가족간 갈등이 심할수록 노인들의 우울이 증가함을 알 수 있었다. 다섯째, 가족친밀감은 스트레스에 음(-)의 영향(p＜0.01)을 주는 것으로 분석된 바, 가족친밀감이 높을수록 노인 우울은 감소함을 알 수 있었다. 결론적으로 노인의 자아존중감, ADL, 스트레스 요인은 노인 우울에 직접적인 영향을 주는 요인으로 나타났으며, 사회적 지지, 가족간 갈등, 가족친밀감 요인은 노인우울에 간접적인 영향을 주는 요인으로 분석되었다. 이러한 결과에 근거하여 정신적으로 건강한 노인을 위해서는 자아존중감 증진, 가족간 갈등 해소와 가족 친밀감 향상을 통한 정서적 지지 확보가 가장 중요하다고 본다.

영문 초록

To identify the impact of abuse, Self-Esteem, ADL, Psychosocial Factors on elderly depression, the survey was conducted on 1,000 elderly people in Seoul and Kyunggi-do from November 8 to November 26 in 2010. Data analyses were carried out using SPSS Ver, 12.0 and AMOS 7.0 software. The major findings of this study are as fellows; First, activities of daily living(ADL) had a positive impact on depression (p＜0.01), and stress as psychological factors had a positive impact on depression(p＜0.01). Second, the elderly abuse had a negative impact on self-esteem(p＜0.001), and self-esteem had a negative impact on depression. In conclusion, the severer elderly abuse, the higher depression. Third, social support had a negative impact on depression(p＜0.001). The result showed that the higher the social support of the elderly, the lower depression. Fourth, the family conflict had a positive impact on stress(p＜0.01), as a result the higher family conflict, the severer depression in the elderly. Fifth, the family intimacy had a negative impact on depression(p＜0.01), as a result the higher family intimacy, the lower depression in the elderly. In conclusion, self-esteem, ADL, stress factors had a direct impact on elderly depression in the elderly. And social support, family conflict and family intimacy were indirect factors affecting on the elderly depression. Based on the results of this study, the promotion of self-esteem, the resolution of family conflict, and the security of emotional support through family intimacy will be regarded as the most important factors for the mental healthy elderly
