장기요양인정 독거노인의 재가서비스 이용 관련 요인

국문 초록

우리나라는 고령인구가 급속히 증가하는 가운데 혼자 사는 노인가구도 급증하고 있다. 고령화에 따른 건강보장 요구에 대처하기 위하여 노인장기요양보험이 도입되었음에도 불구하고 독거노인의 특성을 고려한 장기요양제도에 관한 연구는 거의 없다는 점에 주목하여, 본 연구에서는 독거노인의 장기요양보험 재가서비스 이용실태, 이용의 차이 및 영향요인을 알아보고자 하였다. 분석자료는 2010년 1월 1일 현재 장기요양 인정자 중 전국의 65세 이상 독거노인 27,004명에 대한 국민건강보험공단의 장기요양 인정조사와 급여자료이다. 분석 결과 장기요양 인정을 받은 독거노인 중 한 가지 이상의 재가서비스를 이용한 경우는 29.2%였는데, 그 가운데 방문요양서비스를 한 번이라도 이용한 경우는 27.2%, 방문요양서비스 이외의 재가서비스를 한 번이라도 이용한 경우는 17.9%였다. 건강보장 자격이 건강보험대상자인 경우, 주수발자가 가족인 경우 재가서비스 이용률이 높았고, 주수발자가 간병인ㆍ자원봉사자인 경우 재가서비스 이용률이 낮았다. 건강 특성별로는 치매, 중풍 및 문제행동이 있거나, 일상생활수행능력(ADL)이 20점 이상인 경우, 관절구축이 7점 이상인 경우 재가서비스 이용률이 낮았고, 만성질환수가 1개 이상, 수단적 일상생활수행능력(IADL)이 23점 이상, 상하지운동장애가 6점 이상인 경우 재가서비스 이용률이 높았다. 방문요양 서비스 이용여부에 영향을 미치는 결정요인은 재가서비스 이용여부에 영향을 미치는 결정요인과 별 차이가 없었다. 방문요양서비스 이외의 재가서비스 이용률은 건강보험 경감대상자인 경우 낮았으며, 농어촌 독거노인인 경우 높았다. 건강 특성별로는 치매, 장기요양동급, 문제행동, ADL, IADL, 만성질환수, 관절구축, 상하지운동장애 등이 방문요양 이외 서비스 이용률에 유의한 영향을 미쳤다. 독거노인의 건강상태가 장기요양 서비스 요구도에 미치는 영향은 다가오고 있는 노령사회에서 독거노인의 중가에 따른 장기요양제도 발전을 위한 정책 방향 설정에 중요한 요인의 하나로 고려할 필요가 있다.

영문 초록

This study is to analyse related factors of in-home service utilization in Korean Long-term Care Insurance among certificated elderly living alone. Subjects of analysis were 27,004 certificated elderly living alone who were 65 years and over at the time of Jan. 1, 2010. According to the analysis, 29.2% of the subjects utilized in-home services of long term care insurance, and 27.2% of them home-visit care service and 17.9% the other in-home services, and the rates of overall in-home services were increasing among the eligibilities of health insurance and the elderly with family caregivers. Among the health characteristics, dementia, stroke, and behavioral problems, ADL of 20 points or more, the joint construction of 7 points or more, were significant variables to decrease the utilization rates of in-home services, however the factors of 1 or more of the chronic diseases, IADL of 23 points or more and upper and lower motor impairment of 6 points or more makes the utilization rates increasing. The rate of home-visit care service utilization were similar to the overall in-home services utilization. The utilization rates of in-home services except home-visit care were decreasing among the recipients who get financial help from government, and increased among the rural elderly living alone. The utilization rate of the in-home service except home-visit care were decreased among the elderly with dementia, class 3, behavioral problems, ADL of 23 points or more, the joint construction of 9 points or more, while increased among the elderly with 3 or more of the chronic diseases, IADL of 23 points or more and upper and lower motor impairment of 6 points or more. In conclusion, among the certificated elderly living alone, the utilization rate of in-home service were related with their health characteristics and the other general characteristics. And these findings may be helpful for the establishment of long term care policy for the elderly living alone through the coming aged society.
