의료전문직의 이윤추구 동기가 의료재정정책과정에 미치는 영향

국문 초록

본 연구는 의료전문직의 경제적 이윤추구 동기와 내용이 공공여론과 공식적인 의료재정 정책과정에서 어떻게 논의되고 반영되는지 분석한 것이다. 이윤추구 동기의 원인을 개인성향, 조직문화, 사회환경의 세 가지 설명으로 분류, 검토하였다. 의약분쟁 당시, 관련단체 임원 중심의 심층면담 자료와 정부 관련기관의 내부자료를 분석하였다. 분석 결과, 모든 단계에서 의료계의 이윤추구 동기는 사회환경에 근거한다는 주장이 강하게 대두되었다. 의사 개개인의 경제욕구와 성향에 대한 주장은 최종 정책결정 단계에 이르러 크게 감소하였다. 한편 비공식적 과정에서의 논의나 주장과는 달리, 공식 의료재정정책과정에서 의료단체의 이윤추구 동기와 현상에 대한 논의 자체는 극히 적어 이 동기가 의료재정정책에 미치는 영향은 사실상 미미한 것으로 나타났다. 그러나 내용을 분석해보면, 수가인상 수준에 대한 단체별 이견, 약품실거래가상환제에 대한 다른 시각, 약가 리베이트가 여전히 존재한다는 주장이 모든 단계에서 공통적으로 나타났다. 당시 제기되었던 이슈가 현재 의료체계에서도 여전히 중요한 정책과제임이 주목된다.

영문 초록

This article purports to analyze the medical profession s profit-seeking motivation and behaviour, which are expected to influence the official process of medical financial policy. The study attempts to classify the physicians economic motivation into three explanations: a character explanation, a professional organizational and cultural explanation and a social environmental explanation. Internal documents from the National Health Insurance Corporation and the Ministry of Health and Welfare as well as the in-depth interview data of decision makers concerned during the conflict over the Separation Scheme were employed. Conclusions indicate that the social environments accounted for most of the medical profession s economic behavior all through the phases, from public opinion to the official decision-making processes. In the last official process of medical financial policy, physicians personal characters little explained physicians profit-seeking behavior. In the two government official processes, the proportion of discussion on the medical profession s economic interests was only small. However, the discourse implies that a few issues, such as the level of medical fees and rebates of medicines from pharmaceutical companies, would be longstanding challenging tasks, which turns out to be true.
