국문 초록

본 연구는 계약제가 도입된 2000년부터 2007년말까지 의료보험수가 결정과정에서 나타난 관련 단체들의 복잡하고 역동적인 관계를 검토하여 정치경제학적 의미를 살펴보기 위한 것이다. 수가결정의 내부적이고 구체적인 과정을 검토하기 위해 보건복지부와 국민건강보험공단의 내부자료와 관계자들에 대한 심층 면담 자료를 활용하였다. 분석 결과, 보험가입자들이 수가결정 과정에 가입자대표로 공식적 제도적으로 참여하게 된 것은 주목할 만한 변화이며 현재의 삼자구도 결정틀과 주체들 간의 동수 인원비율을 변경시키기는 상당 기간 어려울 것으로 판단된다. 수가협상에서 제시되는 수치들 이면에는 각 단체의 기본 가치와 명분이 자리하고 있으며 이를 추구하는 데 다양한 전략이 동원되고 있었다. 객관적인 자료와 합리적 기준은 단체간의 불신으로 제대로 생성, 활용되지 못하고 있으며 정치적 합의나 표결이 중요한 해결책이 되고 있다. 정부는 공급자와 가입자 간의 중재자나 조정자 이상의 역할을 하고 있지만 특정한 정책 목표와 가치를 달성하기 위해 내용적으로까지 상황을 주도하는 것에는 이르지 못하는 것으로 판단된다.

영문 초록

This article purports to analyze the dynamic process of setting fees for medical services in the National Health Insurance. Internal documents from the Ministry of Health and Welfare and National Health Insurance Corporation as well as the in-depth interview data of decision makers concerned were employed. The analysis of the decision-making process for medical fees(2000~2007) indicates that: The insured officially joined the Health Insurance Policy Committee, the main institution of setting fees for medical services, as the Payer Group, The Supplier Group and the Payer Group were all dissatisfied with the structure of the Committee, where the Public Interest Group handled the situations. Inside and outside the Committee, the representative groups strove for their own purposes with different strategies. Owing to distrust among the representatives, it was hard to reach an agreement on a research method for a rational level of fees. Instead the political negotiation or a compulsory vote was employed for a solution, The government appeared to have difficulty in achieving its long-term health care policy goal in terms of setting medical fees, in spite of its leading role between the Supplier Group and the Payer Group.
