영국의 보건의료서비스 우선순위 결정 경험과 함의

국문 초록

참여정부에 들어 건강보험의 보장성 강화의 문제가 보건정책의 주요한 이슈로 떠오르고 있다. 건강보험의 보장성 강화를 위하여 보건의료서비스의 우선순위를 합리적으로 설정하여 급여수준을 확대해 나아가야 함은 건강보험의 당면과제라 할 수 있다. 그러나 이 분야의 연구가 국내에서는 매우 부족한 실정인데 비해 선진국에서는 많은 연구가 진행되었다. 따라서 외국의 사례 분석을 통하여 한국의 정책결정에 도움이 될 수 있는 정책적 교훈을 도출할 필요가 있다. 이러한 맥락에서 본 연구는 영국의 보건의료서비스 우선순위 결정 경험을 분석하고 우리나라 실정에 맞는 정책적 합의를 도출하는데 연구목적을 두었다. 영국의 우선순위 결정의 특징은 우선순위를 결정함에 있어 비용-효과성을 중시하고 일반대중의 의견을 반영하려고 노력하는 점이다. 또한 지역보건당국의 우선순위 결정의 재량권이 강하여 지역간 급여수준의 차이가 심화됨에 따라 영국 정부는 NICE를 설립하여 지역간 급여수준의 차이를 해소하려는 노력을 기울이고 있다. 영국의 사례에서 배워야 할 정책적 교훈은 첫째, 임상적으로 효과가 있거나 비용-효과성이 있는 서비스 제공을 위하여 지침을 마련하는 기관과 개발된 지침이 효율적으로 시행되는지 집행 점검하는 기관을 설립하여 임상적 효과성 및 비용-효과성이 있는 서비스가 실제로 국민들에게 제공될 수 있는 체계를 구축할 필요가 있다는 점이다. 둘째, 우선순위 결정을 함에 있어서 의사들의 전문가적 의견이 가장 중요하며 일반대중의 의견을 반영하는 것도 필요하다는 점이다. 일반대중의 의견을 수령하는 방법으로는 일반대중이 우선순위를 결정하기 전 충분한 정보와 시간이 제공되는 시민 배심원제를 채택할 필요가 있다. 또한 의사의 견해와 일반대중의 의견이 상이할 가능성이 높으므로 이들 간의 이견을 어떻게 조율하여 정책결정에 반영할 것인지가 중요하다. 마지막으로 본 연구에서는 우선순위 결정에서 사회적 합의가 필요한 부분을 기술하였다.

영문 초록

In the participatory government, strengthening the security of health insurance has become a significant issue in health policy making. Strengthening the security of health insurance by means of enlarging the health insurance benefit package through rational priority setting in health services is a task Korea s health insurance is currently confronted with. However in contrast to the developed countries, where research in this area has been strenuously conducted, little has been done in the research field in Korea. Therefore it is necessary to learn from foreign case studies, as this will contribute to health policy making in Korea. In this context this paper aims at finding policy implications suited to Korea s situation by carefully analyzing the case of health priority setting in the NHS. Major characteristics setting priorities in Britain are the importance of cost-effectiveness and efforts to reflect the public s view in policy making. Additionally the NHS has pursued to end the postcode lottery caused by continuation of respect for the autonomy of purchasers in their local priority setting, by establishing NICE. What can we learn from Britain? Firstly, in order to build a system in which the public is provided with effective treatments and cost-effective services, the establishment of an organization, which draws up guidelines for the supply of such health services, and a monitoring organization, which ensures these guidelines are followed in the process of supplying health services to the public, is necessary. Secondly, when setting priorities, it is desirable to set priorities according to the professional view of doctors which is most important. In addition, efforts to reflect the public s view in priority setting are also needed. To elicit public opinion effectively, it is necessary to adopt citizens juries, where sufficient information and time is given before the public decides on priority setting. Also as it seems highly likely that some disagreement on priorities between doctors and the public will be revealed, how policy makers can dissolve this disagreement between the two groups will be an important issue. Lastly, this paper discusses the sector of priority setting where social concurrence is needed.
