국문 초록

전국민으로 대상범위를 확대한지 9년째를 맞는 우리 나라의 의료보험제도는 아직도 해결해야 할 많은 문제점들을 가지고 있다. 현행 의료보험제도의 가장 근본적인 문제점으로는 1) 불필요한 의료비의 증가, 2) 높은 자부담률과 낮은 급여수준, 3) 보험 및 조합간 재정격차, 그리고 4) 조합규모의 영세성을 들을 수 있다. 이러한 문제점들을 근본적으로 해결하기 위한 방안으로 1) 비용의식을 높이기 위한 효과적인 공급자 지불방식의 도입과 2) 조합의 규모를 점진적으로 확대해 나아가는 두 가지 정책방안을 생각할 수 있다. 이 두 가지 정책방안 각각을 기대되는 효과의 크기에 따라 세분하여 조합하면 일련의 선택 가능한 개혁정책안을 만들 수 있다. 어떠한 개혁정책안을 선택하여 현행 제도를 개선해 나아갈 것인가에 대한 결정은 반드시 각각의 정책안이 가져올 수 있는 득과 실에 대한 실증적 연구를 바탕으로 이루어져야 할 것이다. 단순히 이념적인 논리에 근거한 개혁을 시도하기에는 우리 나라의료보험제도는 이미 국민들의 복지에 너무도 큰 영향을 미칠 수 있는 보편적인(universal) 사회보장제도로 자리잡게 되었다는 사실을 인식해야 할 것이다.

영문 초록

Since 1989 when the nation’s Health Care System expanded its scope to the entire population, the system has been the most universal social security program in the nation. The nation’s Health Care System, however, still has many problems. Those problems include: 1) rapidly increasing health care costs, 2) high levels of co-payment, yet low levels of benefit, 3) a wide range of differences in financial stability among insurance societies and insurance programs, and 4) having too many small-sized insurance societies. To address these problems at the most fundamental level, two reform strategies are suggested One of them is to enhance cost-consciousness among health care providers by introducing more effective provider-payment schemes. The other is to expand the size of insurance societies. In particular, the latter strategy is expected to bring about a range of positive effects in addition to economies of scale in the operation of the system. Each reform strategy has a range of policy alternatives, and some of them are more effective than others yet more difficult to implement depending upon the degree of changes they might bring about. By combining two policy alternatives, one from each of the two strategies, a range of reform policy alternatives can be developed To envision the right direction toward which the nation’s current health insurance system should evolve, it is imperative to thoroughly weigh the benefit and cost of these policy alternatives, and such comparison should be based on empirical data.
