국문 초록

목 적 : 원위 요골 골절 환자의 요골수근 관절강과 요골 원위부의 연골하골을 관찰하기 위해 시행 되는 Wrist lateral radial tilt view 검사에서 자체 제작한 보조기구 사용의 유용성을 평가하고자 한다. 대상 및 방법 : 연구대상은 2014년 2월부터 2014년 3월까지 손목에 이상 병변이 없는 정상 성인 16명의 오른쪽 손목을 대상으로 하였다. 연구방법은 첫째 아크릴로 제작한 직삼각형 모형의 보조 기구 위에 대상자의 손목을 정측면으로 위치, 둘째 손목을 테이블면에 밀착하고  X선관을 다리쪽 23  입사, 셋째 각도측정을 생략하고 고정기구 없이 손목을 임의로 23  거상, 넷째 대상자의 반대측 손목으로 전완 중앙 아래를 지지하고 검사측 손목은 정측면으로 위치하여 중심 X선을 수직입사하여 촬영하였다. 영상의 평가는 PetaVision을 사용해 요골수근 관절강의 간격을 측정하였으며, Kruskal-wallis test와 Mann-Whitney로 유의성 분석을 하였다. 결 과 : 검사법에 따라 요골수근 관절강의 간격차이를 분석한 결과, 보조기구를 사용한 검사법에서 평균 2.14mm로 가장 길게 나타났다. X선관을 다리쪽으로 23  적용한 경우 1.85mm, 반대측 손목을 지지한 경우 1.25mm, 임의로 23  거상한 경우 1.07mm로 측정됐다. 4개 검사법간의 평균은 통계적 으로 매우 유의한 차이를 보였다(p<0.05). 사후분석 결과  보조기구를 사용한 검사법과 나머지 검사법은 통계적으로 유의하였고(p<0.05), 보조기구를 사용한 촬영법을 제외한 나머지 검사법은 각 각 통계적으로 유의한 차이가 없었다(p〉0.05). 결 론 : Wrist lateral radial tilt view 검사에서 보조기구의 사용은 요골수근 관절강의 간격이 가장 넓게 측정되어 다른 검사법보다 진단적 가치가 높았다. 또한 환자의 상태의 따라 쉽게 적용가능하 고 검사시간의 단축으로 환자와 검사자의 만족도를 높일 수 있으리라 사료된다.

영문 초록

Purpose : To evaluate the usefulness of the device with a wrist lateral radial tilt view in order to observe the radial subchondral bone and radiocarpal joint space of the distal radius fracture. Materials and Methods : Research subjects were directed to the right wrist of 16 healthy adults without abnormal lesions on the wrist from February 2014 to March 2014.Research methods were, first, locate the wrist in the lateral position on the device of right triangle form produced in acrylic, second, locate the wrist in the lateral position on the table and apply X-ray tube angle at caudal 23 ,  third, skip angle measurement and randomly tilt the wrist at 23  without fixed equipment, and forth, support the inferior of the central forearm with the opposite wrist and locate the examining hand in the lateral position, and then project the central X-ray, perpendicularly. Evaluation of the image for the radiocarpal joint space with the PetaVision, and the evaluation of significance was processed with  Mann-Whitney and Kruskal-wallis test. Result :  Results of analysis on the width of radiocarpal joint space with different radiographic techniques indicated that using the device showed the widest as much as 2.14mm, applying X-ray tube angle at caudal 23  was 1.85mm, supporting opposite hand was 1.25mm, randomly tilting the wrist at 23  was the narrowest as much as 1.07mm. The mean points of the four groups indicated statistically significant difference(p<0.05). The difference between the radiographic technique with the device and the rest of the other techniques existed from the Result of post-hoc analysis (p<0.008) on the significance level of the subgroup, and the rest radiographic techniques except for the technique with the device indicated statistically no significant difference (p>0.008).  Conclusion : The device used during a wrist lateral radial tilt view has a high diagnostic value with the widest gap in radiocarpal joint space compared to other techniques. Also, it is readily applicable in accordance based the condition of the patient, and increase the satisfaction from the radiologist and the patient due to reduced time for the test.
