쇄골 골절 환자의 변형된 사방향 촬영법의 유용성

국문 초록

목 적 :쇄골 골절은 성인 골절의 2~5%를 차지하는 비교적 흔한 골절 중 하나로 중간 부위에서 약 69~82%가 골절되며, 외측 부위에서 약 21~28%, 내측 부위에서 약 2~3% 정도 발생 한다. 쇄골의 정규검사인 전후방향, 사방향 검사와 비교하여 중간부, 원위부 쇄골골절 환자의 변형된 사방향 검 사의 유용성을 알아보고자 하였다. 대상 및 방법   : 2013년 5월부터 2013년 12월까지 원위부 쇄골골절로 인해 응급실에 내원한 초진 환자와  쇄골골절 수술을 받은 환자를 대상으로 쇄골 정규 검사인 쇄골 전후방향, 쇄골 사방향을 기본으로 하고 변형된 사방향 촬영을 비교하여 정형외과 교수 2명, 전공의1명외 방사선사 2명이 블라인드 테스트를 통해 총 5점 척도로 유용성을 평가하였다. 변형된 검사법은 환자의 전후방향을 기준으로 골절된 부분의 체간을 약 40도 거상하였다. 입사각은 머리 쪽으로 약15도 입사하여 촬영하였다. 결 과   : 서울대병원에서 시행하고 있는 쇄골골절의 정규검사인 전후방향, 사방향으로 검사한 영상과 비교하여 변형된 검사방법을 시행한 결과 간부, 원위부 쇄골골절의 경우 정규검사에 비해 간부 1.25 ± 0.24, 원위부 1.25 ± 0.12, 골절의 형태와 전위상태는 0.95 ± 0.14, 내 고정술 수술환자의 경우 쇄골의 정렬 상태, 플레이트와 스크류의 위치 는 1.25 ± 0.24정도의 진단적 유용성을 보였다. 반면에 근위부에서는 –1 ± 0.12 으로 정규 검사에 비해 진단적 유용성이 저하됨을 보였다. 결 론 : 일반적으로 쇄골골절은 비수술적 방법으로 대부분 치료되어왔다. 하지만 최근 들어 기존에 추산되었던 것보다 많은 수의 불유합, 부정유합 발생이 보고되면서 수술적 치료의 적응증이 확대되는 추세이다. 따라서 중간부, 원위부 골절의 정확한 진단을 위해 변형된 사방향 촬영법은 쇄골 골절의 수술 전 후 평가에 유용성이 있을 것으로 사료된다. 하지만 근위부의 골절 환자의 경우 체간과 겹쳐서 묘사가 어려워 진단을 내리기에 부적합 하다는 결과가 나타났다.

영문 초록

Purpose : Fractures of clavicle is relatively common fractures accounting for 2~5% of adults fractures, it occurs 69~82% on middle site, 21~28% on outer region, 2~3% on inner site. For investigating the usefulness of modified oblique method for patients with fractures of clavicle on proximal and distal region, comparing test was done to regular X-ray method of clavicle which is AP, Oblique and the modified oblique method. Materials and Methods : Clinical research has been done from the May, 2013 to December, 2013. Among the Patients who had been in emergency department with clavicle fractures surgery and new patients. They had an examination of regular clavicle series order and the modified one. Two orthopedists, a resident and 2 radiation technologists had evaluated the usefulness of the research and taken marks by criterion on 5 point through blind tests. Patients fractures site was lifted 40 degree at AP position level. An incident angle of X-ray was 15 degrees to cranial. Result : The modified method showed 1.25±0.24 of diagnostic usefulness on proximal region, 1.25±0.12 on distal region, 0.95±0.14 on the form of fractures and displacement, 1.25±0.24 on the location of the plate, screw and alignment condition of clavicle for internal fixation. On the other hand, on the proximal site couldn t make it progress, it appeared – 1±0.12 compared with regular clavicle series. Conclusion : In general, nonoperative method was done for clavicle fractures. However, nonunion and vicious union have been more reported than estimated before, operative method is being extended. Therefore, for making an accurate diagnosis of clavicle fractures on proximal distal site, modified method is considered useful evaluating operative management. But, in case of proximal site, it is hard to describe specific clavicle conditions because clavicle’s body is overlap with proximal site.
