골반 단순 전후방 검사에서 내회전 20°를 위한 보조기구의 유용성

국문 초록

목 적 : 고관절의 통증을 주소로 내원하는 환자에게 일차적으로 시행되어지는 방사선학적 검사 중 기본적으로 검사하는 골반 단순 전후방 촬영(Pelvis AP)에서 고관절을 20° 내회전시켜 고정시킬 수 있는 보조기구를 사용하여 검사한 영상을 비교하여 그 유용성을 알아보고자 하였다. 대상 및 방법 : 2012년 7월부터 2013년 10월 까지 고관절 통증으로 본원에 내원하여 기본적으로 촬영하게 되는 골반 단순 전후방검사(Pelvis AP)을 추적 검사한 성인남녀 35명을 대상으로 하였고, 연령군은 남성 17명, 여성 18명 이었으며 총 평균 나이는 47.1세 이었다. 영상분석은 동일한 환자군을 대상으로 보조기구를 사용하기 전과 사용 후의 Femoral offset 측정은 정형외과전문의 1명과 방사선사 1명이 측정 하였고, 소전자의 대칭성, 미골과 치골결합의 일치성, 장골능의 대칭성에 대 한 3가지 질문에 대한 평가는 정형외과파트 방사선사 5명이 리커트 5점 척도로 실시하였다. Femoral offset은 SPSS 20.0 통계프로그램으로 평가자간의 신뢰도인 피어슨 상관분석과 Cronbach의 알파분석을 사용 하였고, 보조기구 사용 전, 후의 차이를 알아 보기위해 대응표본 T검정을 실시하였다. 설문지 평가는 평가자간의 신뢰도를 알아보는 스피어만 상관분석과 Fisher의 정확한 검정으로 p< .05를 유의한 차이로 분석을 하였다. 결 과 : Femoral offset의 평균값은 보조기구사용 전에는 41.74(±.86)mm이고 사용 후에는 43.37(±.96)mm로 사용 전, 후의 평균값 간에는 통계적인 유의한 차이가 있었다(p=.001). 보조기구의 사용여부에 따른 영상기준 평가 3가지(A, B, C) 질문에서 5점을 받은 빈도가 유의한 차이가 있는 것으로 나타났다(P=.000). 보조기구를 사용하기 전 경우에 설문 5점을 받은 경우가 12.4%(13명)이고, 보조기를 사용한 후에 설문 5점을 받은 경우가 46.7%(49명)으로 높게 나타났다. 결 론 : 골반 단순 전후방 검사(Pelvis AP)에서 보조기구의 사용은 고관절 통증으로 환자들의 자세 불안정에 따른 재검사와 검사 보조자에게 불필요한 피폭을 감소시키는데 도움을 주었고, 환자에게 심리적인 안정과 신뢰감을 주었으며 진료과에는 정확한 자세의 검사영상을 전달함으로써 상호보완적인 시스템이 구축되어 보다 더 양질의 의료서비스 제공에 많은 도움이 될 것으로 사료 된다.

영문 초록

Purpose : We sought to find the usefulness of Pelvis Anterior-Posterior (AP) which is a basic test among primary radiology tests performed on the patients who visit the hospital with pain in their hip joints by using an assistive device that can fixate hip joint 20° internally rotated. Materials and Methods : The subjects were 35 patients who had follow-up with Pelvis AP which is a basic test done when patients come in with pain in their hip joints from July, 2012 to September, 2013. Their age group was comprised of 17 male and 18 female, overall mean age was 47.1. Femoral offset is analyzed by orthopedic surgeon 1, radiologic technologist 1, and 3 questions (A, B, C) are analyzed on lickertis 5 scale by orthopedic radiologic technologist 5. Femoral offset is evaluated by the SPSS 20.0 to learn the reliability between evaluators from the Pearson correlation and Cronbach s ㅁ analysis. The difference between before and after using the deivce is verified by Paired T test. To learn the reliability between evaluators in question assessment, Spearman correlation analysis and Fisher s exact test are analyzed by p <.05 significant difference. Result : There was statistically significant difference between the average values of femoral offset before and after using the device (p=.001). Before use, it was 41.74 (±.86)mm, and 43.37(±.96)mm after use. In the 3 questions (A, B, C) in the evaluation of image standards based on the use of device, frequency of answering 5 in the survey appeared to have significant difference (P=.000). The frequency of answering 5 in the survey was 12.4% (13 patients) before using the device and was 46.7% (49 patients) after the usage, which is higher than the former. Conclusion : The assistive device did indeed help to decrease the incidence of retest due to the instable patients and unnecessary exposure of test assistants to radiation. The usage of assistive device provided emotional stability and sense of trust to the patients and test images of more precise position to the diagnosis department.
