요가행법이 월경통에 미치는 영향에 대한 국내 임상논문 고찰

국문 초록

1990년부터 2014년까지 출간된 국내논문을 한정하여 요가(yoga), 월경통(생리, 월경, 생리통, 월경곤란)으로 하여 두 가지 검색어를 동시에 만족하며 본 연구에 부합하는 논문을 검색 엔진을 통해 단 2편으로 선정된 논문을 고찰하였다. 고찰 결과 1) 적절한 피험자 수를 산출하는 과정이 묘사되지 않았다. 2) 연구가 단기간에 이루어져서 장기적인 효과는 알 수 없었다. 2편의 논문 모두 유의한 호전 결과를 제시하고 있으나 요가행법의 좀 더 문헌적인 근거 부족과 여러 동작으로 구성된 프로그램을 썼기 때문에 구체적으로 어떤 동작에 의해 효과가 났는지 파악하기 어렵다. 앞으로 월경통 치료에 있어서 요가의 유효성을 입증할 수 있는 아사나, 호흡법, 명상 등 세분화하여 많은 연구가 이루어져야 하고 이를 위해서는 우선 요가행법의 효과를 객관적으로 평가 할수 있는 기준에 대한 연구가 선행 되어야 할 것이다. 또한 요가행법 종료 후 추후 관찰이 이루어진 경우는 없었고 윤리성 제고를 위한 IRB통과 등 관련연구 설계에서 미흡한 점으로 파악 되었다. 향후 근거중심의학의 측면에서 높은 가치를 지닌 결과물을 도출하기 위해서는 월경통 관련 연구 설계과정에서 1) 전향적 연구 여부, 2) 대조군 연구 여부, 3) 피험자 수와 산정 근거, 4) 무작위 배정 여부와 방법, 5) 맹검 여부와 방법, 6) 주 치료법 내용과 근거, 7) 부가 치료 여부와 효능, 8) 평가 척도, 9) 탈락자 처리 기준, 10) 추후 관찰 여부와 방법, 11) 통계분석 방법, 12) 예상 부작용과 처치 및 보상대책이 필요하고, 논문 작성시에는 1) 연구 설계에 따른 연구 과정의 기술, 2) IRB통과 여부와 시기명시, 3) 연구 일정의 명시, 4) 연구 흐름도에 의한 연구 과정의 도식화, 5) 탈락자 사유 분석, 6) 부작용 유무와 유형 및 인과관계 분석, 7) 타당한 경과 분석과 관련 연구와의 비교, 8) 추후관찰 결과의 분석과 관련 연구와 비교 9) 연구 결과 해석의 제한점 제시, 10) 연구의 시사점과 향후 연구 방향성 제시가 필요하겠다. 월경통 관련 연구 설계과정에서 이러한 점이 보완되면서 논문 기술 과정의 방법론이 정확한 기술과 결과의 정확한 해석이 중시되어야 할 것으로 본다. 현재까진 절대적인 연구의 수가 적은 것도 큰 문제점일 수 있으나 앞으로 좀 더 다양한 요가행법 체계를 임상연구를 통해 그 효과가 밝혀진다면 요가의 다양한 행법들을 월경통을 호소하는 여성들에게 일상생활에 불편함 없이 가정 내에서 안전하고 편리하게 대상자 스스로 적용할 수 있다는 장점이 있다고 본다.

영문 초록

This essay studies two theses selected among the published domestic theses from 1990 to 2014 because they are content with key words, Yoga and Dysmenorrhea. As a result. they do not 1) describes about producing process of the proper number of examinee and 2) we do not know the long term effects because their studies had been made concluded a short period of time. Both of them show the significant results but expose the lack of literature ground of yoga practices and they applied the program composed with many movements so it could not grasp which movements were effect on the dysmenorrhea. In the future the studies have to be accomplished the treatment for the dysmenorrhea which can prove the effectiveness of āsana, praṇayama, meditation and so on. For these, first there will be a leading study for the basis evaluating the effect of yoga practices objectively. There are not any cases of observations after the examination of effects of yoga practice and also we grasp the insufficiency on the planning of involving researches for example a passage of IRB and the forth for increasing the ethicality. After this, in order to deduce the high valued consequences, there will be needed whether there are 1) some forward studies, 2) control group, 3) the number of examinee and the basis of estimate, 4) random assignment and the methods, 5) blinded experiment and the methods, 6) the contents of main treatment and the ground, 7) additional treatment and efficacy, 8) the measure of assessment, 9) the handling standard of people who are not selected, 10) later observation and its methods, 11) the methods of a statistical analysis, 12) the expected side effects and the treatment as well as the compensation in the planing process of studies related with dysmenorrhea. When writing the essay, there will be needed 1) the description of research process according to research planing, 2) whether it passes the IRB and specifies the time, 3) specifies the research schedule, 4) the diagraming of the research process according to the flow of the research, 5) the analysis of the reason for not selected people, 6) with or without side effects and some pattern as well as the causation, 7) the comparison between the proper analysis of the progress and the involving research, 8) the comparison between later observation result and the involving research, 9) describing the limitation of the interpretation on the result, 10) suggesting the thought-provoking of the research and the direction from now on. In the planing process of the research related with dysmenorrhea, it takes seriously the methodology in the essay description course with correct description and correct interpretation of the result. From this study we can apply the various yoga practices to the women who complain the dysmenorrhea.
