비증식당뇨망막병증 환자에서 범망막광응고술 시행 후파열된 망막대동맥류 1예

국문 초록

목적: 비증식성 당뇨망막병증을 진단받은 환자에서 범망막광응고술 시행 후 기존의 망막대동맥류가 악화된 증례를 경험하였기에 이를 보고하고자 한다. 증례요약: 70세 여자가 당뇨 망막병증 검사를 위해 내원 후 시행한 안저 검사 및 형광안저혈관조영 촬영상 양안에 심한 비증식성 당뇨 망막병증(Severe non-proliferative diabetic retinopathy)에 상응하는 소견이 관찰되었고 우안 상이측의 시신경과 가까운 혈관궁 부위에 매우 작은 크기의 대동맥류가 있었으나 크기가 작고 황반 중심부에서 거리가 멀며, 출혈 및 삼출액 등의 증상이 없어 범망막광응 고술 시행 후 경과관찰하였다. 레이저 시행 2개월 후 급격한 시력저하를 주소로 재내원하여 시행한 안저 촬영에서 유리체 혼탁 및출혈, 망막하 출혈이 의심되었고 빛간섭단층촬영상 망막하 출혈 및 망막하 삼출액이 관찰되었다. 기존의 망막대동맥류의 파열에 의한 출혈로 생각하고 유리체내 베바시주맙 주사 및 C 3 F 8 가스 주입술, 유리체 절제술을 시행하였다. 출혈 흡수 후 대동맥류 주변으로 망막 장벽 광응고술을 시행하였으며 이후 시력이 호전되었고 수개월 이상 안정적인 상태를 유지하고 있다. 결론: 범망막광응고술을 시행한 후 무증상의 망막대동맥류가 파열된 경우를 경험하였다. 당뇨망막병증 환자에서 범망막광응고술이 망막대동맥류를 악화시킬 가능성이 있으므로, 레이저 치료 계획 전에 망막대동맥류의 존재를 세심히 확인하는 것이 필요할 것으로 생각된다. <대한안과학회지 2017;58(5):606-610>

영문 초록

Purpose: To report a case of deterioration of a retinal arterial macroaneurysm after panretinal photocoagulation (PRP) for diabetic retinopathy. Case summary: A 70-year-old woman visited our clinic for evaluation of diabetic retinopathy. Fundus examination and fluorescein angiography showed severe non-proliferative diabetic retinopathy and PRP was planned for the patient. In addition, the patient was found to have a retinal arterial macroaneurysm on the superotemporal area of the retina on her right eye. However, the lesion was small and was located far from the macula, causing no symptoms; thus we decided to observe the patient over a period of time. Two months after PRP, the patient revisited the clinic complaining of acute visual loss. Fundus examination showed vitreous and retinal hemorrhage and optical coherence tomography revealed subretinal fluid in the corresponding area. This was considered to be due to aggravation of the pre-existing macroaneurysm. Intravitreal Bevacizumab injection, C 3 F 8 gas injection, and pars plana vitrectomy were performed. After absorption of the hemorrhage, barrier photocoagulation was performed around the retinal macroanerysm. The visual acuity improved and the retina remained stable through the most recent follow-up. Conclusions: We experienced the rupture of a preexisting retinal arterial macroaneurysm in an asymptomatic patient after panretinal photocoagulation. PRP in diabetic patients could aggravate retinal arterial macroaneurysms. Therefore, it is necessary to carefully examine the patient for retinal macroaneurysms when planning a PRP for diabetic retinopathy. J Korean Ophthalmol Soc 2017;58(5):606-610
