사회복지사의 사전연명의료의향서 지식 및 임종의료결정 도움 경험 등에 관한 비교 연구 -노인요양병원과 장기요양 입소시설을 중심으로-

국문 초록

본 연구는 노인요양병원과 장기요양 입소시설 사회복지 사의 사전연명의료의향서 지식 및 임종의료결정 도움경험 등을 측정하고, 두 집단 간의 차이점을 분석하는데 그 목 적이 있다. 노인요양병원 사회복지사 104명, 장기요양 입 소시설 사회복지사 297명, 총 401명을 대상으로 설문 조사 를 실시하였으며, 수집된 모든 자료는 SPSS 21을 사용하여 기술적 통계 방법인 평균, 표준편차, 빈도 분석을 실시하였 다. 또한 두 집단 간 사전연명의료의향서 지식과 임종의료 결정 도움경험 등의 평균 차이 검증, 사회복지사 개인 및 시설 변수에 따른 사전연명의료의향서 지식의 평균 차이 검증을 위하여 T-Test 분석을 하였다. 사회복지사의 사전연명의료의향서 지식 중에서 의료지식 의 경우 노인요양병원(2.20±0.87)과 장기요양 입소시설 (2.26±0.91)간 서로 유의미한 수준에서 차이를 보였다 (p<0.05). 하지만 두 집단 간 통계적인 차이가 있음에도 불 구하고, 두 집단 모두 환자나 가족에게 사전연명의료의향 서에 관한 정보를 제공할 정도의 안전한 서비스를 제공하 지 못하고 있음을 알 수 있다. 사회복지사의 도움경험에서 도 노인요양병원(1.80±0.30)과 장기요양 입소시설 (1.36±0.31)간 유의미한 수준에서 차이를 보였다(p<0.001). 노인요양병원에서는 종교(p<0.05), 교육경험(p<0.001), 연 명의료 도움경험(p<0.001), 사전연명의료의향서 도움경험 (p<0.05), 근무규정이(p<0.05)이 있다고 응답한 사회복지사 의 사전연명의료의향서 지식이 높았으며, 장기요양 입소시 설에서는 교육경험(p<0.001), 연명의료 도움경험(p<0.05), 사전연명의료의향서 도움경험(p<0.05)이 있는 사회복지사 의 사전연명의료의향서 지식이 높았다. 본 연구 결과를 바탕으로 사회복지사가 노인환자와 가족 에게 안전하고 정확한 사전연명의료의향서 정보를 제공하 기 위하여 관련 교육을 강화하고, 표준 지침서를 개발하여 전문적인 도움을 줄 수 있도록 제언하는 바이다.

영문 초록

This study aimed to analyze the differences in a social worker s knowledge of advance life sustaining care directives and experience of aids in the process of end of life care decision making between geriatric hospital and long term care facility in South Korea. The study employed a quantitative research method of collecting data from 401 social workers with a structured questionnaire. Data were analysed by descriptive statistics, paired t-test, using SPSS 21.0 program. The finding showed significant differences(p<0.05) in their medical knowledge of advance life sustaining care directives between those who work at geriatric hospitals(2.20±0.87) and at long term care facilities(2.26±0.91). Even though the study found the statistic differences in their knowledge, their knowledge is not sufficient to provide secure service to elderly patients and their family in both settings. There were also significant differences(p<0.001) in their experience to aid the end of life care decision making process between at geriatric hospital(1.80±0.30) and at long term care facilities(1.36±0.31). In geriatric hospital, the social workers level of knowledge significantly differed based on their religion(p<0.05), education(p<0.001), prior experience in assisting life sustaining treatment decision making(p<0.001), in giving information for advance life sustaining care directives(p<0.05), and the existence of regulatory standards at an institution for end of life care decision making(p<0.05). In long term care facility, the social workers level of knowledge significantly differed based on their education(p<0.001), prior experience in assisting life sustaining treatment decision making(p<0.05), in giving information for advance life sustaining care directives(p<0.05). This study proposed the importance of the social workers knowledge in assisting elderly patients and their family members in both setting, suggesting the higher requirement of education and the regulations to provide safe service for elderly patients and their family caregivers.
