동종 조혈모세포이식 후 눈물막과 안구표면의 초기 변화

국문 초록

목적: 동종 조혈모세포이식 전 안구건조증의 유병률 및 이식 후 눈물막과 안구표면의 초기변화를 알아보고자 하였다. 대상과 방법: 본 연구는 총 28명의 56안에서 시행되었으며, 3차 대학 병원에서 동종 조혈모세포 이식술을 시행 받은 환자들을 대상으로 하였다. 모든 환자들은 동종 조혈모세포이식을 시행 받기 전, 시행 받은 후 1개월, 2개월, 3개월까지 총 4차례 안과 검사를 시행하였으며 방문 시마다 전안부 검사를 포함한 정밀 안과 검사를 시행 받았다. 그리고 안구표면질환지수(ocular surface disease index) 설문지를 통해 안구건조증 여부 및 안구표면에 대한 변화를 평가하였다. 주요 결과 측정치는 최대교정시력, 눈물막파괴시간, 각막염색 점수, 쉬르머검사, 눈물 오스몰농도 그리고 안구표면질환지수였다. 결과: 총 56안 중 40안(71.4%)에서 안구건조증이 이식 전 이미 있었던 것으로 나타났다. 조혈모세포이식을 시행 받기 전 이미 안구건조증 진단을 받은 군에서 눈물 오스몰농도의 경우 기저치에 비해 이식 시행 1달, 2달, 3달 후 통계학적으로 유의하게 증가한 것으로 나타났다(p<0.01). 조혈모세포이식을 시행 받기 전 안구건조증이 없던 군에서는 눈물 오스몰농도의 경우 기저치에 비해 이식 후 2개 월 때 통계학적으로 유의하게 증가된 소견을 보였으며(p=0.01), 눈물막파괴시간은 이식 후 3개월 때 기저치에 비해 통계학적으로 유의하게 감소한 소견을 나타내었다(p=0.02). 그 외 안구표면 수치들은 통계학적으로 유의한 변화를 보이지 않았다. 결론: 동종 조혈모세포 이식을 시행한 환자들에 있어서 100일 이내의 급성기에는 쉬르머검사나 안구표면질환지수 등의 안구표면수치들의 유의한 변화는 없었으나, 눈물 오스몰농도의 급성기 변화가 있었던 점으로 볼 때 이식 후 1-2개월 후부터 안과 검진을 시행하는 것을 권장할 수 있으며, 또한 이식을 받기 전 안구건조증이 있을 경우 더 적극적으로 치료하는 것이 필요할 수 있겠다. <대한안과학회지 2016;57(11):1706-1713>

영문 초록

Purpose: To evaluate the prevalence of dry eye in patients before allogeneic hematopoietic stem cell transplantation (aHSCT) and changes in ocular surface in the acute stage after aHSCT. Methods: We evaluated 56 eyes of 28 patients after aHSCT at a tertiary hospital. All patients underwent a full ophthalmic examination at 1 month before aHSCT (baseline) and 1, 2 and 3 months after aHSCT and answered the ocular surface disease index (OSDI) questionnaire to assess ocular involvement in the form of dry eye syndrome or any other ocular manifestation at each visit. Subjects were divided into 2 groups depending on the presence of dry eye at baseline. The main outcome measures were best-corrected visual acuity, tear break-up time, corneal fluorescein staining, Schirmer test, tear osmolarity and OSDI questionnaire. Results: Dry eye was already present in 40 eyes of 20 patients (71.4%) suffering from hematological disease before aHSCT. Tear osmolarity was significantly increased at 1, 2 and 3 months after aHSCT compared with baseline in the dry eye group (each p < 0.01). Tear osmolarity also increased at 2 months after aHSCT and tear break-up time decreased at 3 months after aHSCT, which were statistically significant (p = 0.01 and p = 0.02, respectively). Other changes in ocular surface indices were not statistically significant. Conclusions: In the acute stage, changes in ocular surface indices such as Schirmer test and OSDI were not statistically significant. However, significant changes in tear osmolarity in both groups indicate that ophthalmic examination 1 or 2 months after aHSCT is recommended. Additionally, aggressive treatment is warranted when patients have dry eyes at baseline. J Korean Ophthalmol Soc 2016;57(11):1706-1713
