두 종류의 일체형 비구면 인공수정체 삽입 수술 후 장기 임상 결과의 비교

국문 초록

목적: 백내장 수술 시 반짝임 변성 방지 일체형 비구면 인공수정체 삽입 수술 후 장기 임상 결과를 기존의 일체형 비구면 인공수정체와 비교 분석하고자 하였다. 대상과 방법: 반짝임 변성 방지 일체형 비구면 인공수정체인 enVista MX60을 삽입한 38명의 38안(A군)과 기존에 널리 이용되는 일체형 비구면 인공수정체인 AcrySof IQ를 삽입한 46명의 46안(B군)의 의무기록을 후향적으로 분석하였다. 술 후 그리고 3, 6, 12개월째의 최대교정시력, 대비감도검사, 인공수정체 반짝임 변성, 목표 굴절값에 대한 구면대응치의 오차, 후낭혼탁 및 주관적인 빛번짐 증상을 비교하였다. 결과: 두 군 모두에서 술 후 12개월째 통계학적으로 유의한 최대교정시력 상승과 대비감도 증가를 보였다(p＜0.001). 술 후 12개월째 인공수정체 반짝임 변성은 A군에 비해 B군에서 통계학적으로 유의한 증가가 있었으나(p=0.028), 대비감도검사상 공간 빈도에 따른각 열에서의 최소역치점수 및 주관적 빛번짐 증상은 두 군 간 유의한 차이를 보이지 않았다. 두 군에서 술 후 12개월째 목표굴절값에 대한 구면대응치의 오차는 각각 -0.38 ± 0.27D, -0.36 ± 0.28D였고, 후낭혼탁이 발생한 경우는 각각 2안, 4안이었다. 결론: 반짝임 변성 방지 일체형 비구면 인공수정체인 enVista MX60은 기존의 일체형 비구면 인공수정체인 AcrySof IQ에 비해 수술후 12개월째의 반짝임 변성은 유의하게 적었으나, 최대교정시력, 대비감도 및 주관적인 빛번짐 증상 등 시력의 질적인 면에서는 차이가 없었다

영문 초록

Purpose: To compare the clinical outcomes of glistening-free intraocular lens (IOL) and conventional 1-piece aspheric IOL in implanted in-the-bag. Methods: After phacoemulsification performed by a single surgeon, 2 different IOLs were implanted: enVista MX60 (glisteningfree 1-piece aspheric IOL) in 38 eyes (group A) and AcrySof IQ (conventional 1-piece aspheric IOL) in 46 eyes (group B). Glare symptom score (0-5) obtained by questionnaires, best corrected visual acuity (BCVA), Functional Acuity Contrast Test (FACT), posterior capsular opacity (PCO), glistening formation and spherical equivalent error were compared and analyzed preoperatively and 6 months and 12 months postoperatively. Results: A statistically significant improvement of BCVA and contrast sensitivity postoperatively was observed in all groups. There was statistically significant increase of glistening in group B compared with group A. However, there was no significant difference of FACT scores of spatial frequency in A, B, C, D and E columns and glare symptom score (0-10) obtained by questionnaires 12 months after surgery. Spherical equivalent errors were -0.38 ± 0.27 D and -0.36 ± 0.28 D for groups A and B, respectively. PCO occurred in 2 eyes in group A and 4 eyes in group B. Conclusions: EnVista MX60 IOL showed less glistening formation than AcrySof IQ IOL, however, there was no significant difference in terms of vision quality such as BCVA, FACT and glare symptom score at 12 months postoperatively.
