국문 초록

연구배경: 노화에 따른 신체 기능 장애에 대한 조기 징후로 근력의 저하가 신체 기능 저하와 연관성이 있다는 연구들이 있지만 아직 우리나라 노인들을 대상으로 시행한 연구는 없었으며, 특히 객관적이고 정량적으로 신체 기능 평가를 하지 못했다는 제한점이 있었다. 이에 본 연구는 우리나라 한 지역사회에 거주하는 외견상 건강한 노인들을 대상으로 일상생활 기능 수행에서 중요한 신체의 일부인 손의 악력과 신체 기능의 변화와의 관련성을 알아보고자 한다. 방법: 지역사회에 거주하는 거동 및 일상생활이 가능한 60세 이상의 노인들 중 인지기능에 영향을 줄 수 있는 병력을 가지고 있는 경우는 연구대상에서 제외시켜 총 77명의 노인들을 대상으로 시행하였다. 기본 문진, 신체계측과 혈액검사 및 우울은 Geriatric Depression Scale (GDS), 인지기능은 Mini Mental State Examination (MMSE)를 이용하여 측정하였다. 또한 악력(hand grip power)을 측정하였고 보행기능, 하지 근력, 평형 기능으로 신체활동능력을 평가하였다. 결과: 신체 기능 지수와 악력과의 상관분석 결과 양의 상관관계를 보였고(r=0.49, P＜0.0001), 다른 혼란변수들을 통제한 후 다단계 다중회귀분석을 시행하였을 때, 신체 기능 지수는 악력, 나이, 혈중 cortisol과 독립적인 관련성을 보였다(각각 β=0.14, P＜0.0001; β=−0.11, P=0.0006; β=−0.20, P=0.03). 결론: 다른 혼란변수들을 보정한 후에도 악력과 신체 기능 지수와 독립적인 양의 관련성이 있음을 나타내었으며, 이는 임상에서 간단하게 악력의 측정만으로 노인 신체 기능 저하를 평가할 수 있음을 시사한다.

영문 초록

Background: There are some studies that declined muscle power emerged as an early markers for age-related physical disability. However those studies had a limitation that they did not use quantitative and objective tools in assessing physical function. Furthermore there has been no study conducted in the Korean elderly. This study aimed to investigate the association between hand grip power and declined physical function in community dwelling elderly. Methods: A total of 77 community dwelling apparently healthy old people who can carry out daily life independently were recruited in this study. History taking, blood sampling and physical examination were obtained. Depression was assessed by the 15-item geriatric depression scale (GDS), and cognitive function was examined by the Mini-Mental State Examination (MMSE). We also measured hand grip power and physical performances (gait speed, chair-stand times, tandem standing times). Results: Hand grip power was positively correlated with physical performance score (r=0.49, P＜0.0001). After adjustment of confounding factors using step-wise multiple regression analysis, hand grip power, age and serum cortisol levels were significantly associated with physical performance score (β=0.14, P＜0.0001; β=−0.11, P=0.0006; β=−0.20, P=0.03, respectively). Conclusion: Hand grip power was independently associated with physical performance score. The result suggests that the declined physical function in the elderly may be assessed easily by measuring hand grip power in clinical setting.
