노인생활시설의 케어리스크 매니지먼트 체계구축을 위한 탐색적 연구

국문 초록

본 연구는 노인수발보험제도의 실시와 함께 노인생활시설에서 중요한 이슈로 부각될 케어리스크 매니지먼트(care risk management) 체계구축을 위한 탐색적 연구이다. 본 연구에서는 노인생활시설의 주요 케어리스크 요인인 상해 및 사망사고의 발생현황과 케어리스크 예방프로그램, 시설종사자들의 케어리스크 발생 불안감 및 케어리스크 매니지먼트 체계구축에 필요한 사항들을 살펴보았다. 자료는 2006년 2월 21일과 22일 양일에 걸쳐 노인복지시설협회에서 개최한 세미나에 참석한 시설장과 실무자 135명을 대상으로 수집되었다. 연구결과에 의하면 2005년 한 해 동안 절반 이상의 요양시설과 80%에 가까운 양로 및 전문요양시설에서 상해사고가 발생한 것으로 밝혀졌다. 또한 다섯 시설 중 한곳에서는 사망사고가 발생한 것으로 드러났다. 케어리스크 발생과 관련하여 시설장은 물론 실무자들도 책임소재 여부 등으로 케어리스크 발생에 대해 불안감을 가진 채 근무하고 있는 것으로 나타났으며, 케어리스크가 발생했을 때 시설에서는 노인과 그 가족을 설득하여 문제를 해결하는 경향이 높았다. 각 시설의 케어리스크 매니지먼트를 위해서는 예산확보, 전담직원 배치, 매뉴얼 및 교육프로그램 개발이 중요한 것으로 조사되었으며, 노인복지시설협회가 주축이 되어 케어리스크 매니지먼트 체계를 구축해 나가는 방안이 모색되었다.

영문 초록

This study intends to elicit basic data for developing a better care risk management system for long-term care facilities. Statistic investigation on the occurrences of accidential injuries and death, prevention program for care risk, anxiety experienced by staff related to care risk and other helpful elements to make a better care risk management system are included in this study. Information and data have been collected from 135 members of management personnels and direct care providers who attended at the seminar held between February 21 and 22, 2006, under the auspices of the Korea Association of Senior Citizens Welfare Institutions. Major findings were as follows: Accidental injuries were happened in 68.6% and accidental death had been occurred in 20.4% of long-term care facilities. These accidents have caused uneasiness, insecurity, guilty feeling and responsibility to both management personnels and direct care providers. Consequently, the management personnels and direct care providers feel unstable in their duties and there is a tendency that when accidents happen the management personnels usually negotiate with the family members of the troubled seniors to deal with their complains and possible lawsuits. Effective care risk management system requires significant funding, care risk manager, care risk management manual and prevention programs for improving the quality of care and services. There is a need for further research, but the present results should be the first step in developing care risk management system in long-term care facilities for the elderly.
