병원에 입원한 노인의 영양상태에 관련된 요인

국문 초록

연구배경 : 본 논문은 지금까지 노인 영양상태 평가의 지표로 알려진 여러 요소를 바탕으로 병원에 입원한 노인환자들의 영양상태를 살펴본 것이다. 방법 : 이 조사는 1997년 12월 1일부터 1998년 7월 10일까지 서울과 구미의 순천향병원에 입원한 65세 이상 노인환자 191명을 대상으로 직접 인터뷰를 하여 이루어졌다. 성별, 배우자 유무, 술, 담배, 식사횟수, 신체질환 유무와 간이 영양 평가(Mini-Nutritional Assessment, MNA)와의 관계는 카이제곱 분석을 통하여 상호 연관관계를 판명하였다. 나이, 노인 우울증 척도(Geriatric Depression Scale, GDS), 간이 정신 상태 검사(Mini-Mental State Examination, MMSE), 사회망 척도(Social Network scales), 도구적 일상생활활동(Instrumental Activities of Daily living,IADL), 기본적 일상생활활동(Basic Activities of Daily living,BADL) 등과 MNA의 관계는 상관분석과 카이제곱 분석을 이용하였다. 그리고, 상관분석 혹은 카이제곱 분석에서 유의한 상관관계가 있는 변수를 독립변수로 하고, MNA를 종속변수로 하여 로지스틱회귀분석을 실시하였다. 결과 : 조사 대상자의 평균 나이는 71.8세였으며, 남자가 101명(52.9%), 여자가 90명(47.1%)이었다. 그 중 남자는 75세까지가 76명(75.2%), 76세 이상은 25명(24.8%)이었으며, 여자의 경우 75세 이하는 68명(75.6%), 76세 이상은 22명(24.4%)이었다. MNA의 평균치는 18.8±0.41정이었으며, 영양불량 상태에 있는 노인 환자는 남녀별로 각각 26명(25.7%), 15명(16.7%)이었다. 결론 : 로지스틱회귀분석 결과 가장 영향을 많이 주는 요소는 하루의 식사횟수였고, 그 다음이 BADL, 우울증, 사회망 척도, IADL, 성별, 심각한 만성질환, 뇌혈관 질환의 순서였으며, 예상 밖으로 치매는 노인의 영양상태에 영향을 주기는 하지만, 그 정도가 미미하였다. 이상에서 우리가 중점적으로 분석하려 했던 운동성에서는 BADL과 IADL이 강하거나 혹은 뚜렷한 상관관계를 보임으로써 역시 노인의 영양상태와 관련성이 높다고 할 수 있다.

영문 초록

Background : This study aimed to research the nutritional conditions of the aged patients admitted in the general hospital on the basis of various elements known till now as the factors for nutritional assessments of the aged. Methods : This research was done via direct interview & chart review with 191 aged patient over 65 who are in both Seoul & Gumi hospital of Soonchunhyang University from December 1, 1997 to July 10, 1998. The co-relation between gender, living alone or not, alcohol, smoking, meal per day and severe chronic disease and Mini-Nutritional Assessment (MNA) was made clear through Kai-square Analysis. The relation between age, Geriatric Dpression Scale (GDS), Mini-Mental State Examination (MMSE), Social Network Scale (SNS), Basic Activities of Daily Living (BADL), Instrumental Activities of Daily Living(IADL), and Mini-Nutritional Assessment(MNA) was researched using ANOVA and Kai-square Analysis. And the Logistic Regression Analysis was practiced regarding the variable that has significant as relation in ANOVA or Kai-square Analysis as independent variable, and MNA as dependent variable. Results : The average age of the subjects was 71.8, and they consist of 101 men (52.9%) and 90 women(47.1%) : for the male, 76(75.2%) were under the age of 75. and 25(24.8%) over 76 : for the female, 68(75.6%) were under the age of 75 and 22(24.4%) over 76. The mean of MNA were 18.8±0.41, and the number of the aged who were suffering from malnutrition were 26(25.6%) and 15(16.7%) respectively. Conclusion : As a result of the Logistic Regression Analysis, "meal per day" was the most influential element, followed by BADL, depression, Social Network Scale, IADL, Gender, severe chronic disease and cerebra-vascular disease. Unexpectedly, the dementia's influence on malnutrition of the aged was very little, if any, As for the Mobility on which we focused to analyse, there was a strong or clear co-relation with BADL and IADL, and therefore, it seemed that it has close co-relation with malnutrition of the aged.
