의학 전문직업성의 덕 윤리적 접근

국문 초록

현대의 의사들은 급속한 생명과학과 생의학의 발달, 상업화와 의료경영의 효율성 중시 등으로 생긴 윤리적인 문제점들에 직면하고 있을 뿐만 아니라 의료보험 제도의 문제 및 의사에 대한 사회의 불신으로 위축되고 있다. 이 같은 복잡계로서의 사회적 맥락 아래서 의사들이 의학 전문직업성의 핵심 가치인 이타심, 봉사정신, 자율성 및 전문직 우위 같은 전통적인 덕목들을 지켜나가는 것이 힘들어지고 있다. 현대의 의학 전문직업성은 치료자와 전문직이라는 야누스적 면을 가지고 있지만 사실 그 뿌리는 덕 윤리이다. 의학 전문직업성의 덕목들을 살펴보면 이타심, 연민, 숙련됨, 책임감, 의무감, 공손함, 책무 및 인격적 통합성 등이 있다. 결국 덕이 있는 의사들의 성향이 이와 같은 요소를 지니고 있기에 의학 전문직업성이 덕에서 기원했음을 알 수 있다. 의료 행위의 본질인 의학 전문직업성에 내포된 덕(Virtue)은 그리스어로 아레테(Arete)인 탁월함, 완전함, 우수함, 훌륭함 및 뛰어남을 의미한다. 아리스토텔레스가 인간이 이성과 사유의 기능을 탁월하게 발휘해서 습관화시킨 것을 덕이라 불렀듯이 개별의사의 의학 전문직업성의 덕목들도 의사들이 꾸준히 노력한다면 향상시킬 수 있다. 하지만 현대는 의료제도가 사회와의 계약으로 존재하므로 미시적인 개별 의사의 의학 전문직업성은 거시적인 사회적 덕에 종속 될 수 밖 에 없게 된다. 그러므로 의사들은 자신의 덕을 함양시킴과 더불어서 실천적 지혜(practical wisdom)를 발휘해서 사회와 정치 제도의 발전과 개혁에도 눈을 돌려야한다.

영문 초록

Doctors living in modern society have to not only face ethical dilemmas arising from biomedicine and life science but also often deal with the flawed medical insurance system. In such social context of complexity, doctors struggle to maintain traditional virtues of medical professionalism such as altruism, service, autonomy and professional priority. Even though medical professionalism mainly focuses on two different aspects of doctors, as healers and professionals, it is essentially rooted in virtue ethics. Some of the most crucial elements of medical professionalism are altruism, compassion, competence, excellence, responsibility and integrity. The aforementioned characteristics are driven by one's virtue; it has been known that virtuous doctors posses such qualities as well. The word virtue stems from the word Arete in Greek, which means excellence, perfectness, outstanding, superiority and nobility. As Aristotle stated that virtues can be expressed by habitual patterns of behavior, the virtues of medical professionalism can also be improved and sustained with practice. In today's society where medical system is bound by societal contract, the virtue of individual doctors is subordinate and interconnected to the virtue of society. Therefore, doctors must not only improve their own virtue but also strive to realize the virtue of society by utilizing their practical wisdom.
