학령기 뇌성마비 아동의 운동기능 분류체계에 따른 기능, 활동 및 참여의 비교

국문 초록

본 연구는 학령기 뇌성마비아동의 운동기능 분류체계(Gross Motor Function Classification System, GMFCS)에 따른 기능, 활동과 참여를 국제기능, 장애, 건강분류의 18세 이하 버전인 국제 기능, 장애, 건강분류–청소년과 유아 버전(International Classification of functioning, disability and health children & youth, ICF-CY)에 의거하여 알아보고자 실시되었다. 연구대상은 7∼12세의 학령기 뇌성마비아동 57명이며, 설문지와 ICF-CY 체크리스트, 대근육 운동기능 평가(Gross Motor Function Measure, GMFM) 및 GMFCS를 이용하여 평가하였다. 연구결과, GMFCS에 따른 GMFM 점수 차이는 경증이 가장 높았고, 중등도, 중증의 순서로 나타났다. GMFCS에 따른 ICF-CY의 기능 항목의 차이는 정신기능, 감각기능과 통증, 음성 및 말하기 기능 항목에서 경증과 중등도는 중증보다 유의한 차이를 보였고, 신경근육골격 및 운동과 연관된 기능들 항목에서 경증은 중증보다 유의한 차이를 보였다. ICF-CY의 활동과 참여 항목의 차이에서는 학습과 지식적용, 일반적인 임무와 요구, 의사소통, 대인관계, 주요생활 영역 항목에서 경증과 중등도는 중증보다 유의한 차이를 보였고, 이동, 자기관리, 가정생활, 공동체 생활, 사회생활 및 시민으로서의 생활 항목에서는 경증이 중등도와 중증보다 유의한 차이를 보였으며, 중등도가 중증보다 유의한 차이를 보였다. 이상의 결과를 볼 때, 학령기 뇌성마비아동의 기능, 활동과 참여는 뇌성마비아동의 GMFCS 차이에 따라 달라지는 것을 알 수 있다.

영문 초록

The purpose of this study was to comparison of the function, activities and participation in school-aged children with cerebral palsy by Gross Motor Function Classification System(GMFCS). The subjects were 57 patients(7~12 years old) school-aged children with CP. The GMFCS, Gross Motor Function Measure(GMFM), International Classification of functioning, disability and health children & youth(ICF-CY) checklist, and questionnaire were used for this study in order to measure children’s function, activities and participation in school-aged children with cerebral palsy. As a result, the GMFM point was significant differences in mild level, compared to moderate level and severe level. The differences in function section of ICF-CY checklist, mental functions, sensory function and pain, voice and speech functions were significant defferences in mild level and moderate level, compared to severe level. neuromusculoskeletal and movement related functions were significant defferences in mild level, compared to severe level. The differences in the activities and participation section of ICF-CY checklist in school-aged children with cerebral palsy in according to GMFCS were statistically significant in entire activity and participation. These result suggest that the school-aged cerebral palsy children functions, activities and participation, depends on the ability of children(GMFCS difference) and functions of children with cerebral palsy affects activities and participation.
