구개파열 아동과 일반 아동의 자음정확도, 언어능력, 읽기능력 및 비음치와 말명료도 간의 관련성

국문 초록

배경 및 목적: 구개파열 아동은 기질적인 문제로 인하여 청자에게 말하고자 하는 바가 충분히 전달되지 않아 의사소통 시 어려움을 겪을 수 있다. 본 연구는 구개파열 아동과 일반 아동간에 말명료도 및 화자요인의 차이를 비교 제시한 후 구개파열 아동의 말명료도와 화자요인간 관련성을 살펴보고자 하였다. 방법: 화자요인은 조음능력(아동용 발음평가, Assessmentof Phonology & Aticuration Children: APAC), 언어능력(구문의미이해력검사), 읽기능력(기초학습기능 수행평가체제, Basic Academic Skills Assessment: BASA, 이하 BASA),Nasometer를 이용하여 측정된 비음치였다. 말명료도 평가는 BASA의 ‘토끼야 토끼야1-(1)’문단읽기를 실시한 것을 일반인 18명이 5점 척도로 평정하였다. 본 연구의 대상으로는 구개파열 아동과 일반 아동 각각 9명씩 총 18명을 대상으로 하였다. 통계분석은 집단 간 말명료도와화자요인 간 차이를 보기 위해 독립표본 t -검정을, 말명료도와 화자요인 간 관련성을 보기위해 피어슨 상관분석과 다중회귀분석을 실시하였다. 결과: 구개파열 아동은 일반 아동에 비해말명료도, 언어능력, 자음정확도가 통계적으로 유의하게 낮았고(p < .05), 비음치는 통계적으로 유의하게 높았으며(p < .05), 읽기능력은 유의한 차이를 보이지 않았다. 말명료도와 화자요인 간 관련성에서는 구개파열 아동과 일반 아동은 모두 읽기능력이 좋을수록 말명료도도 통계적으로 유의하게 좋았다(p < .05). 마지막으로 다중회귀분석을 통해 독립변인인 언어능력, 읽기능력, 비음치와 종속변인인 말명료도와의 관련성을 살펴본 결과, 말명료도는 읽기능력과통계적으로 유의하게 관련되어 있으나 조음능력, 언어능력, 비음치와는 통계적으로 유의한관련성을 보이지 않았다. 논의 및 결론: 본 연구 결과에 따르면 구개파열 아동의 말명료도평가시 읽기능력은 고려되어야 할 화자요인으로 여겨지므로 구개파열 아동의 두드러진 말특성인 조음능력과 공명문제 등과 더불어 언어치료 시 임상에서 다뤄줘야 할 것으로 사료된다

영문 초록

Background & Objectives: Children born with cleft palate have problems in resonance, articulation and language ability. Additionally, cleft palate affects hearing, communication and psychology. The most specific and remarkable trait in these children is speech disorder.The primary cause of articulation and resonance disorders in children with cleft palate is velopharyngeal inadequacy (VPI). Children with cleft palate have lower speech intelligibility because of the VPI. Methods: The purpose of this study was to examine the relationship between speech intelligibility and speaker factors, such as articulation ability, language ability, reading ability, and nasalance score, in children with cleft palate. Eleven children with cleft palate and 11 normal children were included in this study. Results: We found a statistically significant correlation between speech ntelligibility and reading ability in children with cleft palate. However, other variables, including articulation ability, language ability and nasalance score, did not show any significant correlation. Through multiple regression analysis, we found that reading ability is a significant independent variable to speech intelligibility. This study confirms and suggests differences in speech intelligibility and speaker factors between children with cleft palate and normal children. Speech intelligibility, articulation ability, language ability, and nasalance score did not show statistically significant differences. However, reading ability did produce significant differences between the two groups. Discussion & Conclusion: Based on these results, approaches in therapy to improve the reading ability of children with cleft palate could be handled in clinic, and further research on various aspects that showed a significant difference from normal children, such as articulation ability and language ability, could be conducted.
