농촌노인 건강실태 - 나주시를 중심으로

국문 초록

  이 연구는 농촌 노인의 만성질환 유병율과 우울상태, 삶의 질을 파악하여 지역 특성에 맞는 근거 중심의 건강증진사업을 위한 기초자료를 제공할 목적으로 시행하였다. 분석 대상은 다단계 비례할당 추출방법(multi-stage proportionate quota sampling)으로 선정된 45세이상 나주시 면지역민 중 65세 이상 노인 456명이다.
  조사 대상자의 연령이나 결혼상태, 교육수준이나 경제상태 등 사회인 구학적 지표로 볼 때, 농촌과 노인의 특성이 잘 나타났다. 나주시 노인의 신체생리적 특성은 전국 읍？면 노인 평균과 다르지 않았다.
  건강생활 실천실태를 살펴볼 때, 나주시 면지역 노인들은 운동 미실천율과 음주율이 높은 편이고 흡연율은 다소 낮은 것으로 조사되었다.
  응답자의 70.8%가 조사한 만성질환 12가지 중 적어도 한가지 질환에 대하여 약을 복용하거나 병원치료를 받고 있으며, 특히 관절염과 디스크는 전국 읍면 평균 유병율보다 유의하게 높은 것으로 나타났다. 그러나 고혈압, 당뇨병 등의 생활습관질환은 전국 읍면 평균보다 낮은 것으로 조사되었다.
  삶의 질은 노유자(1988)의 도구로 측정하였다. 5점 만점에 평균 2.69점으로 대체로 낮은 편이나 이웃과의 관계(3.46점)나 정서상태(3.08점), 가족관계(3.06점)는 덜 낮은 편이었다. 그러나 경제생활(2.30점)과 신체 상태와 기능(2.39점) 영역의 삶의 질이 낮은 것으로 조사되었다.
  우울증은 노인우울척도(Geriatric Depression Scale: 기백석, 1995)로측정하였다. 응답자의 23.0%가 중증 우울증을 가지고 있는 것으로 나타났다. 이는 매우 높은 수준인데, 경증 우울증까지 합하면 전체의 62.3%가 우울증을 가지고 있는 것으로 나타났다. 나주시 노인의 우울증에 대한 심도있는 조사와 함께 적극적인 중재가 필요한 것으로 생각된다.
  이 결과를 바탕으로 볼 때, 나주시의 건강증진사업은 운동 미실천율과 음주율을 낮추는 방향으로 추진하는 것이 바람직하다. 또한 관절염과 디스크, 우울증을 예방, 관리하는 교육이 필요하다. 이러한 사업과 함께 경제상태를 증진시킴으로서 노인의 삶의 질을 높일 수 있을 것이다.
  이 연구결과는 나주시와 유사한 대부분의 농촌에 조심스럽게 확대하여 적용할 수 있으나, 조사 초기부터 노인을 대상으로 계획하여 자료를 수집한다면 더욱 좋은 결과를 얻을 수 있을 것이다.

영문 초록

  The purpose of the study was to provide baseline prevalence of the chronic diseases and depression, and the level of quality of life among rural elderly. Of the 800 subjects over 45 years of age surveyed using multi-stage proportionate quota sampling method from rural Naju-shi, Korea, only 456 elderly over 65 years of age were analyzed in this study.
  Sociodemographic factors such as age, marital status, education, and economic status reveal typical characteristics of the "rural" and the "elderly". Physical-physiological characteristics of the respondents were not different from the Korean average of the rural elderly. Elderly of Naju-shi exercise less, smoke less and drink more than average Korean rural elderly.
  Of 456 elderly, 70.8% take medicine or receive treatment for at least one of the 12 chronic diseases we surveyed. The prevalence of arthritis and that of herniated lumber disc(HLD) were statistically significantly higher than national average of rural Korea. The prevalence of high blood pressure and that of diabetes were significantly lower.
  Quality of life(QOL) was measured using Noh(1988). The average QOL score was pretty low(2.69 of 5.00). Of the 6 areas of QOL, neighborhood relationship(3.46), emotional status(3.08), family relationship(3.06) were relatively higher. However, economic status(2.30) and physical status and function(2.39) were relatively lower.
  Depression was measured using Geriatric Depression Scale(GDS; Ki, 1995). Sixty-two percent was found to have mild to severe depression. This figure was higher than most any survey results published so far. Twenty-three percent of the subjects showed severe depression. We recommend more vigorous and in depth survey to accurately evaluate and properly intervene depression in rural Naju-shi.
  Based on the results, we suggest that the local health promotion should focus on promoting excercise and reducing drinking. We also suggest health education aiming at prevention and management of arthritis, HLD, and depression. QOL of the elderly of Naju-shi can be increased by upgrading their income level along with the implementation of the health promotion programs.
  The results can be carefully extended to other rural areas of Korea. However, better results can be obtained if data collection is targeted to the elderly from the planning stage of the survey.
