삼킴곤란 환자에게 복합 중재로 적용된 신경근 전기자극치료의 효과에 대한 체계적 고찰

국문 초록

목적 : 본 연구는 국내와 국외의 신경근 전기자극치료와 다양한 치료를 결합한 중재의 효과와 적용 방법에 대해 분석하고자 하였다. 연구방법 : 2014년 1월부터 2023년 12월까지 Riss, 국회도서관, 구글 학술검색, Pubmed, Science direct를 통하여 검색하였다. 주요 검색 용어는 ‘연하 & 전기자극치료’ / ‘삼킴곤란 & 전기자극치료’ /삼킴장애 & 전기자극치료’ / ‘Dysphagia & electrical stimulation’을 사용하였고, 총 9편의 문헌을 분석 대상으로 선정하였다. 결과 : 총 9편의 문헌 중 연구 대상군은 급성기, 만성기를 포함한 뇌졸중 환자가 가장 많이 차지하였고, 선정된 9편 중 8편의 문헌에서 다양한 치료와 결합한 신경근 전기자극치료 중재가 삼킴기능 증진에 유의미한 효과를 보였으며, 1편의 문헌은 유의미한 효과를 보이지 않았다. 결론 : 본 연구에서 신경근 전기자극치료와 다양한 치료를 결합한 중재에 대하여 체계적으로 고찰하며 중재의 효과와 적용 방법을 확인하였다. 추후 다양한 질환군을 원인으로 삼킴곤란을 겪는 환자들에게 다양한 치료를 결합한 신경근 전기자극치료가 활발히 이루어져야 할 것이며 다각도의 측면에서 더욱 다양한 연구가 이루어져야 할 것이다.

영문 초록

Objective : This study aimed to analyze the effectiveness and application of interventions combining neuromuscular electrical stimulation with different treatments at home and abroad. Methods : Ten-year studies were searched from January 2014 to December 2023, using Riss, the Library of the National Assembly, Google Academic Search, PubMed, and Science Direct datasets. “Swallowing and electrical stimulation therapy,” “difficulty in swallowing or dysphagia,” and “electrical stimulation therapy” were the primary search terms. Nine documents were selected for analysis using a 5-step qualitative evidence level analysis model. Results : Among the nine studies, patients with stroke, including acute and chronic strokes, accounted for the majority of the study group. In eight of the nine selected studies, neuromuscular electrical stimulation interventions combined with disparate treatments significantly improved swallowing function, whereas one study showed no significant effect. Conclusion : This study systematically reviewed an intervention combining neuromuscular electrical stimulation and various other treatments and confirmed the effectiveness and method of intervention application. Future research should further examine NMES interventions in different patient populations with swallowing impairments.
