영문 초록

Polymyositis is an inflammatory myopathy characterized by subacute muscle weakness. Respiratory muscle invasion is rarely observed in patients with polymyositis. Guillain-Barré syndrome (GBS) is an immune-mediated polyneuropathy characterized by acute respiratory failure. A 32-year-old woman with a four-day history of general weakness and myalgia is presented herein. Serum creatine kinase (CK) and creatinine levels were elevated. Based on the suspicion of rhabdomyolysis with acute kidney injury, the patient underwent continuous renal replacement therapy. However, her muscle weakness worsened and she experienced dyspnea. Electrodiagnostic test results and patient symptoms led to the suspicion of GBS. However, the patient’s serum CK level increased despite intravenous immunoglobulin administration. Muscle biopsy and electromyography were performed, confirming polymyositis. After prednisolone administration, the patient’s extremity muscle strength gradually improved. Therefore, muscle biopsies and electromyography should be conducted in patients with suspected GBS to rule out inflammatory myositis, including polymyositis.
