아동학대로 인한 선택적 함묵증 아동의 놀이치료 사례연구

국문 초록

본 연구는 양육자에 인한 아동학대로 인해, 불안한 환경과 유아기 박탈경험으로 인해 선택적 함묵증을 보이는 아동의 사례이다. 아동은 양육자와의 불안정 애착으로 욕구 충족의 어려움과 부정적 자아상을 보이고 있으며, 세상에 대한 신뢰 부족 및 사회적인 관계에 어려움을 보이고 있다. 본 연구에서는 놀이상담과 모래놀이상담을 통하여, 아동이 안정적으로 보호받는 치료적 관계에서 애착의 재경험 형성과 자신의 에너지를 표현할 수 있도록 하였다. 주 1회 50분 동안 총 59회기에 걸쳐 놀이치료 52회, 모래놀이치료 7회가 진행되었다. 치료초기, 아동은 마음속에 격렬한 에너지를 적절하게 표출하지 못하고 자기 자신을 지키며 언어 표현의 어려움을 보였으나, 놀이치료를 통하여 자유롭게 자신을 감정을 표현하고, 그 감정을 언어적으로 표현함으로써 상담자와의 안정적인 신뢰관계를 형성할 수 있었다. 또한. 놀이가 진행됨에 따라 아동의 표현력이 성장했다. 따라서 말을 시키기 위해 조급함을 보이기보다 놀이를 따라가며 안정적인 경험을 할 수 있는 환경을 만들어 주는 것이 중요함을 보여주었다. 아동의 선택적 함묵 증상은 강압적이고 지시적인 父에 대한 반항적 거부 형태인 수동－공격적 함묵으로 이해하고 父와의 관계에서 아동 행동의 이해를 돕도록 개입을 함께 하였다. 상담적용 결과 아동의 행동상의 변화와 센터교사의 보고를 종합적으로 봤을 때 함묵 증상의 변화와 사회적 관계에서의 변화를 보고하였다.

영문 초록

This study presents the case of a child with the selective mutism arisen from insecure living environment and child abuse during early childhood by a caregiver. The child’s insecure attachment with the caregiver caused difficulties in satisfaction of needs, development of negative self-image, lack of trust in society and difficulty in social relationships. The study allows the child to re-experience attachment and express his energy within secure and protected therapeutic relationships via play therapy and sand play therapy. Total Fifty nine sessions composed of fifty two play therapy sessions and seven sand play therapy had been progressed and a session time was fifty-minute in a week. In beginning stage of therapy process, the child was not able to suitably express his energy and seemed to be in self-protecting stage having difficulty in expression. However the child was able to form stable relationship with a therapist and allow unconstrained feeling and verbal expression through play therapy. In addition, as the play therapy progressed, the child expressiveness appeared outstanding. It demonstrate the importance of accompanying child’s play and keeping therapeutic environment secure. That is to indicate a therapist is necessary to accept child’s aggression and have energy to withstand. The symptom of child with selective mutism is understood as child’s passive-aggressive mutism which is rebellious refusal behaviors toward oppressive and instructive father. Therefore, the therapist intervened the father to understand the child’s behaviors within father’s relationship with the child. As the result of therapy, child’s overall mutism symptom and his social relationship are changed according to teacher’s report and change of the client’s behaviors in the center.
