한국인의 도구적 일상생활활동(IADL)의 수행 빈도, 중요도 및 관련 요인에 대한 연구

국문 초록

목적: 본 연구는 한국 성인의 도구적 일상생활활동의 수행 빈도와 중요도를 알아보고, 도구적 일상생활활동에 영향을 미치는 관련 요인을 확인하여 향후 한국인의 도구적 일상생활활동 관련 연구의 기초자료를 제공하고자 하였다. 연구방법: 2016년 2월 1일부터 2017년 7월 30일까지 국내 지역사회에 거주하고 있는 20세 이상 80세 이하 일반 성인 588명을 대상으로 그들이 참여하고 있는 도구적 일상생활활동에 대해 조사하였다. 설문문항은 일반적인 특성 12개 문항과 작업치료실행체계(Occupational Therapy Practice Framework, OTPF) 3판을 기반으로 12개 도구적 일상생활활동에 대한 수행 빈도와 중요도로 구성되었으며 리커트 5점 척도로 조사를 실시하였다. 분석은 수행 빈도와 중요도의 Pearson 상관관계 분석과 일반적인 요인에 따른 수행 빈도와 중요도 차이를 알아보기 위해 t-test와 ANOVA를 실시하였다. 연구결과: 12개의 도구적 일상생활활동 중 한국 성인이 가장 수행 빈도가 높은 것은 ‘식사준비와 치우기’로 4.12±1.08점으로 나타났으며, 중요도는 ‘건강관리와 유지하기’가 4.04±0.89점으로 가장 높게 나타났다. 도구적 일상생활활동의 수행 빈도와 중요도의 상관관계에서는 ‘종교의례’와 ‘안전과 응급상황관리’ 2개 항목을 제외하고는 통계학적으로 유의미한 양의 상관관계를 보였다(p<.05). 한국 성인의 일반적 특성 중 도구적 일상생활활동의 수행 빈도와 관련 있는 요인은 직업, 연령, 교육수준, 약물복용, 음주 및 흡연여부 등의 순으로 나타났으며, 중요도와 관련 있는 요인은 연령, 결혼여부, 교육수준, 성별, 직업, 주소지, 월평균수입 등의 순으로 나타났다(p<.05). 도구적 일상생활활동의 수행 빈도와 중요도는 개인이 가지고 있는 특성에 따라 유의미한 차이를 보였다(p<.05). 결론: 본 연구는 20세 이상의 한국 성인 및 노인의 도구적 일상생활활동을 분석하여 제시하였다는 점에서 중요한 의미를 가지며, IADL 수행에 영향을 주는 개인적, 문화적, 사회적 배경 요소에 대한 정보를 제공함으로써 작업치료사들이 도구적 일상생활활동의 평가와 중재계획을 설계하는데 기초 자료로 유용할 것이다.

영문 초록

Objective : The purpose of this study is to investigate the performance frequency and the importance of instrumental activities of daily living (IADLs) in Korean adults and identify factors influencing them. Methods : Five hundred and eighty-eight adults aged from those in their 20s to those in their 70s who lived in the community were selected as subjects of the study and IADLs in which they participated were investigated. The questionnaire consisted of 12 general characteristics and the frequency and importance of 12 IADLs based on the 3rd edition of Occupational Therapy Practice Framework (OTPF) and the Likert 5 point scale was employed. We investigated the correlation between the frequency and importance of IADLs and the factors related to the frequency and importance of IADLs in Korean adults. Results : Among the 12 IADLs, the highest frequency recorded for Korean adults was meal preparation and clean up (4.12 ± 1.08). The importance of IADLs was highest in relation to health care and maintenance (4.04 ± 0.89). There was a statistically significant positive correlation between the performance frequency and importance of IADLs (p<0.05) except for religious and spiritual activities and expression. Among the general characteristics of Korean adults, factors related to the frequency of IADLs were occupation, age, education level, drugs, and drinking, and factors related to importance were age, marital status, education level, gender, occupation, address, monthly average income and so on (p<.05). There was a correlation between what they regarded as important in the IADLs and the use frequency. The use frequency and importance of the IADLs showed a significant difference according to the characteristics of individuals. Conclusion : It is very important to reflect factors related to the individual s environment and background in the evaluation of the IADL and intervention plans in occupational therapy.
