성인 Ⅲ급 부정교합자의 악안면골격구조 및 치열궁형태에 관한 연구

국문 초록

본 연구는 성인 III급 부정교합 환자에서의 성별, 수직피개도에 따른 악안면골격, 치성구조의 차이 및 치열궁 형태의 차이를 알아보기 위해 시행되었다. 49명의 성인 III급 부정교합자의 측모 두부 X-선 계측사진과 진단모형을 이용하였으며, 측모 두부 X-선 계측사진에서 24개의 선계측 항목과 22개의 각도계측 항목,12개의 비율항목을 선정하였고, 진단모형에서는 상, 하악의 악궁 장경, 폭경, 전치부 총생, 구치관계를 계측 또는 계산하였다. 성별, 수직피개도별 t-tset를 시행하였고, 수직피개도에 따른 t-test 결과 유의차를 보인 항목을 대상으로 하여 수직피개도가 0mm보다 작은 개교군과 0mm보다 큰 비개교군을 나누는 다중판변분석을 시행하였다. 결과는 다음과 같다; 1. 하악기저골 길이와 전상치조고경을 제외한 대부분의 선계측 항목에서 남자가 유의하게 큰 값을 보였으나(P<0.05), 악간관계, 두개저에 대한 상, 하악골의 상대적 위치 항목에서는 성별 유의차가 없었다. 2. 모형계측치에서 상, 하악 전치부 총생, 하악 제 1 소구치간 폭경, 상악치열장경, 평균구치관계를 제외한 항목에서 남자가 유의하게 큰 값을 보였다(P<0.05). 3. 개교군과 비개교군사이에 상, 하악골 크기항목에서는 유의차가 없었으나 개교군에서 하악각의 증가(P<0.05), 하악지의 후방경사(P<0.01)가 유의하게 있었고, 상악골(P<0.01), 하악골(P<0.05)이 모두 두개저에 대한 전방위를 취하였다. 4. 개교군에서 전하안면고경(P<0.05)과 후상치조고경(P<0.05)이 유의하게 컸으며 이는 하악골의 두개저에 대한 위치에 대한 보상적으로 작용하였다. 개교군에서 전상후상치조고경비(P<0.001)와 전하후하치조고경비(P<0.05)는 유의하게 작은 값을 보였다. 모형 계측치중에서는 상악전치부 총생(P<0.05)만이 개교군에서 유의하게 작은 값을 보였다. 5. 성인 III급 부정교합에서 전치부 개교군과 비개교군을 구분하는 다중판별분석식을 구했으며 분석식에 포함된 항목은 하악각, 전하후하치조고경비, 전상후상치조고경비, FH-NA였다. 위 판별식으로 본 연구대상을 판별한 결과, 전체정판별률은 92.86%였다.

영문 초록

This study was conducted to discern-differences of craniofacial, dentnalveolar structure and model measurements between sex and between class lII openbite group and non-openbite group. The sample consisted of 49 adult patients with class Iii malocclusion. 24 linear measurements, 22 angular measurements and 12 ratios were -selected in lateral cephalometry. Also, arch width, length, antericr crowding, average molar relation were measured or calculated in diagnostic model. . The data were evaluated by t-test and multiple discriminant analysis. The results were as follows ; 1. Most linear measurements, with the exception of MnBL and AUDH, were significantly larger in male(p<0.05). but, intermaxillary relations and spatial position of maxilla and mandible relative to cranial base were not different for both sex. 2. With the exception of upper and lower anterior crowding, lower arch width, upper arch length, AMi3, male exhibited significantly larger measurements in model analysis (p<0.05). 3. Size differences of maxilla and mandible between openbite and non-openbite group were not significant(p>0.05). but openbite group showed significantly increased gonial angle(p<0.05), FH-CoGo(p<0.01), FH-NA(p<0.01) and FH-NB, FH-NPog (p<0.05). 4. ALFH and PUDH were larger(p<0.05) in openbite group. this result served as compensation for the spatial position of mandible relative to cranial base. AUPUDH (p<0.001) and ALPLDH(p<0.05) were lower in openbite group. upper anterior crowding was the only measurement which showed difference between openbite and non-openbite group(p<0.05). 5. For the purpose of classifying adult class lII openbite and non-openbite group, multiple discriminant analysis was done. gonial angle, ALPLDH, AUPUDH, FH-NA were included in multiple discrinunant equation. 39 cases(92.86%) were correctly classified when applied to the sample used in this study.
