직업재활이 정신장애인의 과제수행능력과 삶의 질에 미치는 효과: 사례연구

국문 초록

목적 : 본 연구는 만성 정신장애인을 대상으로 의미 있는 직업재활이 환자의 과제수행능력과 삶의 질에 미치는 효과에 대해서 알아보았다. 연구방법 : 연구 대상자는 경상도 소재의 병원에 입원중인 비 기질적 정신장애를 진단받은 46세 여성으로 주 2회－3회, 2달간 자발적으로 직업재활 프로그램에 참여하였다. 직업재활 프로그램은 대상자가 선택한 요리 활동이 포함되도록 구성하였고 원내 직업재활원에서 실시되었다. 직업재활 전후의 과제수행능력과 삶의 질은 AMPS와 COPM을 통해 평가하였다. 결과 : 대상자가 직업재활에 참여한 결과 AMPS의 운동기술 점수는 1.92 logit에서 2.12 logit으로 증가하였고, 처리기술 점수는 1.55 logit에서 2.05 logit으로 증가하였다. COPM에서는 분식가게 운영, 어머니 간호, 봉사활동에 대한 수행도의 평균 점수가 6.7점에서 8.3점으로 향상하였고, 만족도의 평균 점수는 5.3점에서 7.7점으로 증가하였다. 결론 : 대상자가 과제를 선택하고 자발적으로 참여한 직업재활 프로그램을 통해 과제수행능력과 삶에 대한 만족도가 향상될 수 있었다. 자발적 참여와 직업 재활에 대한 만족도는 정신장애인의 직업 유지에 중요한 요인이라 할 수 있다. 따라서 앞으로는 정신장애인이 안정적이고 지속적인 직업유지를 할 수 있도록 취업 전 단계의 준비들과 함께 개인의 정신적 특성에 맞는 다양한 프로그램들이 개발되어야 할 것이다.

영문 초록

Objective : The purpose of this case study was to investigate the effects of vocational rehabilitation a client with mental disorder in task performance and quality of life. Methods : A 46-year-old female inpatient diagnosed with non-organic mental disorder in Gyungsang province. She participated in various vocational rehabilitation programs including cooking group 2-3 times a week. Pre and post performance skills and perceptions on quality of life were assessed with the Assessment of Motor and Process Skills(AMPS) and the Canadian Occupational Performance Measure(COPM). Results : The results showed that the motor skills improved from 1.92 logit to 2.12 logit, and the process skills increased from 1.55 logit to 2.05 logit. Average performance and satisfaction scores of the COPM on managing snack bar, caregiver of mother, and volunteer activity increased from 6.7 to 8.3 and 5.3 to 7.7 respectively. Conclusions : Through vocational rehabilitation programs which participant chose the task performance and satisfaction showed significant improvement. Voluntary participation and satisfaction on vocational rehabilitation are important factors for persons with mental disorders to maintain careers. Therefore, it is suggested that various occupational therapy programs be provided to meet the needs of individual clients with mental disorders in addition to prevocational programs.
