산후 정신장애에 대한 임상적 고찰 ㅡ과거 10년간 고려대학교 부속병원 신경정신과 입원 및 외래환자를 중심으로一

영문 초록

The author studied seventy six female patients who developed their mental illnesses within the 3-month period after childbirth and were treated at the neuro-psychiatric department of Korea University Hospital from March 1971 to May 1981 The average age at onset was SO. 3 years, ranging from 18 to 44 years, and the most patients (43. 4%) were between and 25 to 29. Twenty six patients (34.2%) were primipara and fifty patients (65.896) were multipara. Most patients (83.0%) had the onset of their illnesses within the 30-day period after childbirth and 38 patients (50.0%) had the onset within the 1-week period after childbirth. Thirty nine patients (51.3%) were diagnosed as having a neuroses, twenty four patients (31.6%) as having a unipolar depression, twelve patients (15.8 %) as having a schizophrenia, and another one patient (1. 3%) as having a delirium state. Their clinical symptoms were similar to these of mental disorders not associated with childbirth， but characteristicslly 1 case cf infanticide and 1 case of infanticidal attempt were seen. In their sexual life, twenty nine patients (38. 2 %) were unsatisfactory, seven patients (9.2%) of them were frigid and another twenty two patients (28.9%) complained of the feeling of unsatisfactioiu Thirty six patients (47.4%) had poor relationship with their parents in childhood, including nineteen patients (24.0%) of early parental loss before patient’s 12th year. Fourteen patients (18. 4%) had negative attitude toward their baby, four patients (5.396) of them had unwanted pregnancy and another ten patients (13.2%) had their children of unwanted sex. Forty two patients (55. 3%) had the social stress or life event during pregnancy and puerperium. Among these patients, thirteen patients (17.1%) had the illness or death of family member, eight patients (10. 5%) had the financial loss, and twentyone patients (27.7%) had trouble with their husbands or in-laws.
