폐쇄각의증에서 시신경 주변과 황반부의 망막 미세혈관구조

국문 초록

목적: 폐쇄각의증과 정상 대조군에서 빛간섭단층혈관조영술을 이용하여 망막모세혈관인자를 분석, 비교하였다. 대상과 방법: 폐쇄각의증으로 진단받은 42안과 정상 대조군 38안의 시신경유두주변부, 황반부의 혈관밀도, 관류밀도, 중심와무혈관부위를 빛간섭단층혈관조영술을 통해 측정, 분석하였다. 결과: 양 군에서 시신경 주변 망막신경섬유층 두께는 차이가 없었으나 폐쇄각의증에서 시신경주변 표층모세혈관총 하부의 혈관밀도와 관류밀도는 낮았고(각각 p=0.032, 0.026), 신경절세포-내망상층의 두께는 차이를 보이지 않았지만 폐쇄각의증에서 황반부 표층모세혈관총 고리 모양 내측 하부 혈관밀도와 관류밀도, 고리 모양 외측 하부, 코쪽 혈관밀도와 관류밀도가 낮았다(각각 p<0.05). 결론: 망막신경섬유층과 시신경 유두에서 녹내장성 변화를 보이지 않았던 폐쇄각의증에서 정상안과 비교하였을 때 시신경주변과 황반부 미세혈관 하측부의 혈관밀도, 관류밀도는 유의하게 낮았다.

영문 초록

Purpose: To compare the macular and peripapillary vascular parameters between the primary angle closure suspect (PACS) eyes and normal control eyes using optical coherence tomography angiography (OCTA). Methods: In this retrospective study, 42 PACS subjects and 38 normal individuals are included. Vessel density (VD) and perfusion density (PD) of the macular area and peripapillary area, and fovea avascular zone parameter were analyzed using OCTA and compared between two groups after adjustment for the axial length. Results: There were no statistically significant differences in sex ratio, age, refractive errors and intraocular pressures between both groups. In the peripapillary area, inferior VD and PD in the outer zone were significantly lower in the PACS eyes, while the retinal nerve fiber layer thicknesses were similar between groups (p = 0.032, 0.026). In the macular superficial capillary plexus, inferior VD and PD in the inner zone, inferior VD and PD, nasal VD and PD in the outer zone were significantly lower in the PACS eyes, whereas ganglion cell inner plexiform layer thickness were similar between both groups (all p < 0.005). Conclusions: The VDs and PDs of retinal capillaries in PACS eyes, which have no glaucomatous changes in the retinal nerve fiber layer and optic nerve disc, were significantly lower compared to normal eyes.
