비중격만곡 및 비중격교정술 여부가 눈물주머니코안연결술의 경과에 미치는 영향

국문 초록

목적: 비중격만곡과 비중격교정술의 여부가 눈물주머니코안연결술의 수술 후 경과에 미치는 영향을 확인한다. 대상과 방법: 건양대학교병원에서 2015년 3월부터 2022년 1월까지 단안의 원발성 후천적 코눈물관폐쇄로 눈물주머니코안연결술을 받은 환자의 의무기록을 분석하였다. 환자를 비중격만곡이 없었던 군(group 1), 수술 부위와 동측의 비중격만곡이 있는 군(group 2), 이측의 비중격만곡이 있는 군(group 3)으로 나누었다. 동측의 비중격만곡이 있을 경우, 관련 증상이 있는 경우에 한하여 비중격교정술을 시행하였다. 비중격만곡의 유무, 종류 및 비중격교정술 여부가 눈물주머니코안연결술의 수술 후 경과와 연관이 있는지 확인하였다. 결과: 총 204명의 환자가 연구에 포함되었다. Group 1은 94명, group 2는 60명이었으며, group 3은 50명이었다. 각 그룹 간 비중격만곡의 종류, 정도, 눈물주머니코안연결술 시 수술 시간, 성공률 및 수술 후 증상 호전까지 걸린 기간을 비교하였을 때 유의미한 차이가 관찰되지 않았다. Group 2를 비중격교정술을 시행하지 않은 그룹(group 2A)과 비중격교정술을 시행한 그룹(group 2B)으로 나누어 비교했을 때에도 유의미한 차이가 관찰되지 않았다. 결론: 본 연구에서 비중격만곡 및 비중격교정술은 눈물주머니코안연결술의 경과에 영향을 미치지 않았다. 원발성 후천적 코눈물관폐쇄 환자가 비중격만곡으로 인한 증상을 호소하지 않는 경우, 비중격교정술이 반드시 필요하지는 않은 것으로 생각된다.

영문 초록

Purpose: To investigate the impact of nasal septal deviation and septoplasty on dacryocystorhinostomy progression. Methods: From March 2015 to January 2022, we reviewed the medical records of primary acquired nasolacrimal duct obstruction (PANDO) patients who underwent dacryocystorhinostomy at Konyang University Hospital. Patients were categorized into groups: those without septal deviation (group 1), those with septal deviation on the same side as the surgical site (group 2), and those with septal deviation on the opposite side (group 3). Septoplasty was performed in cases of symptomatic septal deviation on the same side. We assessed whether the presence, type, and correction of septal deviation influenced the postoperative outcome of dacryocystorhinostomy. Results: The study included 204 patients: 94 in group 1, 60 in group 2, and 50 in group 3. No significant differences were observed in the type, degree of nasal septum deviation, surgery duration, success rate, and symptom improvement period across the groups. When group 2 was subdivided into those who did not undergo septoplasty (group 2A) and those who did (group 2B), no differences were noted. Conclusions: Nasal septum deviation and septoplasty did not influence the outcomes of dacryocystorhinostomy. Septoplasty may be unnecessary for PANDO patients without symptomatic nasal septum deviation.
