인공수정체 공막고정술 시행 방법에 따른 장기적인 예후 비교: 봉합 및 무봉합 고정법

국문 초록

목적: 인공수정체 공막고정술에 사용되는 대표적인 수술 방법인 봉합 인공수정체 공막고정술과 무봉합 인공수정체 공막고정술의 장기적인 예후 및 합병증을 비교하였다. 대상과 방법: 2019년 1월부터 2023년 6월까지 인공수정체 탈구, 무수정체, 수정체 탈구를 진단받고, 봉합 인공수정체 공막고정술을 시행한 봉합군 46명과 무봉합 인공수정체 공막고정술을 시행한 무봉합군 44명을 분석하였다. 나이, 성별, 안축장 길이, 수술 소요시간, 최대교정시력, 안압, 굴절값, 굴절값 오차, 안구 내 잔류 난시, 재발성 인공수정체 탈구, 인공수정체 기울어짐, 인공수정체 홍채끼임 여부를 분석하였으며, 술 전 및 술 후 1년까지의 변화 양상을 확인하였다. 결과: 술 전 측정된 최대교정시력, 안압, 굴절값은 봉합군과 무봉합군 사이에 유의한 차이가 없었다. 최대교정시력은 술 후 1주와 1개월에 봉합군보다 무봉합군에서 유의하게 개선되었다(p=0.010, 0.015). 굴절값 오차는 술 후 1주에서만 봉합군보다 무봉합군에서 유의하게 작았다(p=0.020). 안구 내 잔류 난시 술 후 모든 시기에서 봉합군보다 무봉합군에서 유의하게 작았다(p<0.001). 수술 소요 시간은 무봉합군에서 유의하게 짧았으나, 분산은 유의하게 컸다(p<0.001, 0.024). 안축장 길이가 길수록 인공수정체 홍채끼임의 발생률이 높았다(p=0.004). 중심황반두께는 수술 전후 두 군 사이에 유의한 차이가 없었다. 결론: 무봉합 인공수정체 공막고정술은 수술 소요 시간이 짧고, 술 후 초기부터 빠른 최대교정시력 개선, 작은 굴절값 오차, 작은 인공수정체 난시를 보인다.

영문 초록

Purpose: This study compared the long-term outcomes and complications associated with sutured and suture-less scleral fixed intraocular lenses (SFIOLs), the two primary surgical techniques for scleral fixation of IOLs. Methods: A total of 90 patients were included in the study with 46 in the sutured group and 44 in the suture-less group. These patients underwent SFIOL between January 2019 and June 2023 due to IOL dislocation, aphakia, or crystalline lens dislocation. Collected data included age, gender, axial length, operative time, best corrected visual acuity (BCVA), intraocular pressure (IOP), refractive value, refractive error, ocular residual astigmatism, IOL dislocation, recurrent IOL dislocation, IOL tilt, and IOL iris capture. Changes from preoperative status to 1 year postoperatively were assessed. Results: No significant differences were found between the sutured and suture-less groups regarding preoperative BCVA, IOP, and refractive values. However, the suture-less group demonstrated significantly better BCVA at both 1 week and 1 month postoperatively compared to the sutured group (p = 0.010, 0.015, respectively). The refractive error in the suture-less group was also significantly lower at 1 week postoperatively (p = 0.020). Ocular residual astigmatism was consistently lower in the suture-less group across all postoperative intervals (p < 0.001). While the suture-less group showed a shorter average surgical time, there was greater variability in operative duration (p < 0.001, 0.024, respectively). Additionally, a longer axial length was associated with an increased incidence of IOL iris capture (p = 0.004). Conclusions: Suture-less SFIOL is associated with shorter operative times, although with greater variability. Patients in the suture- less group experienced faster BCVA improvement, reduced refractive error, and lower ocular residual astigmatism compared to those in the sutured group.
