혈액투석환자의 만성질환관리 자기효능감과 환자역할행위이행의 관계에서 생의 의미의 매개효과

국문 초록

본 연구는 혈액투석환자가 경험하는 만성질환관리 자기효능감과 환자역할행위이행의 관계에서 생의 의미의 매개효과를 규명한 서술적 조사연구이다. 2023년 7월 3일부터 7월 21일까지 구조화된 설문지로 자료수집하였으며 총149명이 참여하였다. 수집된 자료는 기술통계, t-test, ANOVA, Scheffé test, Pearson’s correlation coefficient, multiple regression, Baron과 Kenny 매개효과 검증절차로 분석하였다. 혈액투석환자는 만성질환관리 자기효능감이 높을수록 생의 의미를 높게 인지하고(r=.61, p<.001), 환자역할행위이행 수준도 높은 유의한 양의 상관관계를 나타내었다(r=.59, p<.001). 또한 생의 의미를 높게 인지할수록 환자역할행위이행수준이 높은 유의한 양의 상관관계를 확인하였다(r=.65, p<.001). 생의 의미(β=.38, p<.001)는 만성질환관리 자기효능감과 환자역할행위이행의 관계에서 부분 매개효과를 보여 혈액투석환자의 만성질환관리 자기효능감은 생의 의미를 매개로 환자역할행위이행에 영향을 미치는 것을 확인하였다(Z=4.18, p<.001). 이에 만성질환관리 자기효능감과 생의 의미를 향상시켜, 혈액투석환자의 환자역할행위이행을 증진시킬 수 있는 중재 마련이 요구된다.

영문 초록

This study aimed to examine the mediating effect of meaning in life in the relationship between self-efficacy for managing chronic disease and compliance of patient role behavior in hemodialysis patients. Participants included 149 hemodialysis patients. Data were collected from July, 3 to July, 21, 2023 using self-reported questionnaires. Data were analyzed using descriptive statistics, t-test, ANOVA, Scheffé test, Pearson’s correlation coefficient, multiple regression. Mediation analysis was performed using the Baron and Kenny method and Sobel test. The higher the self-efficacy for managing chronic disease, the higher the meaning in life(r=.61, p<.001), and the higher the compliance patient role behavior(r=.59, p<.001). In addition, a significant positive correlation was found that the higher the awareness of the meaning in life, the higher the compliance of patient role behavior(r=.65, p<.001). Meaning in life showed partial mediating effects in the relationship between self-efficacy for managing chronic disease and compliance of patient role behavior. Thus, it is required to develop interventions to enhance self-efficacy for managing chronic disease and to strengthen meaning in life. It is necessary also to prepare programs and strategies to improve compliance of patient role behavior.
