국문 초록

갑상선기능검사에 이용되는 immunoassay 검사법은 LC-MS/MS 대비 검사 오류가 있다는 점을 인지하고, 환자의 임상 증상이나 투약력도 고려해서 결과를 판정해야 한다. 갑상선기능저하증의 표준 치료는 LT4이며, 심혈관질환, 연령, 임신 등을 고려하여 치료한다. LT4 치료 후에도 증상이 충분히 개선되지 않는 일부환자의 경우 deiodinase나 갑상선호르몬 수송체의 유전적 변이나 갑성선호르몬에 내성이 있는 경우와 더불어 T4에서 T3의 말초변환 장애 및 기타 다른 질병에 대한 가능성도 고려해야 하겠다. LT4에 반응이 없는 환자 중 일부는 LT3 단독 제제, LT4와 LT3 복합 제제, DTE에 증상 개선을 보였다. T3는 미토콘드리아 생합성을 유도하므로 만성 피로나 심근 경색 후 치료 등에 있어서 LT3가 긍정적일 가능성이 있다. 하지만 미토콘드리아 대사 항진에 따른 산화 손상 가능성도 있으므로 LT3의 보충 시작 시기와 용량 조절에 대해서는 추가적인 연구가 필요하겠다. 환자의 개별적인 영양 또는 해독 치료 및 기능의학적 5R 치료는 갑상선호르몬의 흡수나 대사에 영향을 미칠 수 있으므로 LT3가 포함된 제제의 사용과 더불어 기능의학을 이용한 평가 및 치료에 대해 추가적인 연구가 필요하겠다.

영문 초록

Hypothyroidism is a condition in which the thyroid gland does not produce enough thyroid hormones to maintain normal physical activity. Common causes include autoimmune diseases such as Hashimoto's thyroiditis, surgical removal of the thyroid gland, and radiation therapy. It presents with a variety of clinical symptoms, including dry skin, puffiness, intolerance to cold, slowed thinking, and weight gain. However, there are patients who do not show sufficient symptom improvement with LT4 (levothyroxine) alone, the standard treatment for hypothyroidism. As our understanding of thyroid physiology advances, the reasons for discomfort during LT4 monotherapy are becoming clear, including problems with peripheral conversion of T4 (thyroxine) to T3 (triiodothyronine), genetic mutations in deiodinase and thyroid hormone transporter, and thyroid hormones resistance has been studied. In this article, we will look into the characteristics of each thyroid hormone preparation, conventional treatment on hypothyroidism, and thyroid physiology, and present the necessity of LT3 (liothyronine) and functional medicine approach and treatment when treating patients who do not improve with LT4 treatment alone.
