국문 초록

연구목적 : 국내 강력범죄 피해자들을 대상으로 외상초점 인지행동치료(trauma-focused cognitive behavioral therapy, TF-CBT)와 안구 운동 민감소실 및 재처리 요법(eye movement desensitization and reprocessing, EMDR) 치료의 효과를 알아보고자 하였다. 방 법 : 2014년부터 2018년까지 인천스마일센터에 등록된 범죄피해자 중 TF-CBT와 EMDR 치료를 받은 24명 중 치료의 사전, 사후 척도를 시행한 19명의 환자들을 대상으로, 치료 전후의 외상 후 스트레스 진단 척도(Korean version of the Posttraumatic Diagnosis Scale, PDS), 사건충격척도 수정판(Korean Version of the Impact of Event Scale-Revised, IES-R), 벡우울척도 2판(Korean-Beck Depression Inventory-II, BDI-II), 벡불안척도(Korean-Beck Anxiety Inventory, BAI) 결과를 비교분석하였다. 결 과 : 치료 전에 비교할 때, TF-CBT와 EMDR 치료 후 전체 대상자의 평균 PDS, IES-R, BDI-II, BAI 점수가 호전된 것으로 나타났다. 진단별로 살펴보면 전체 대상자 중 PTSD 진단군에서 IES-R 중 회피항목을 제외한 모든 척도에서 유의미한 호전이 확인되었고, 적응장애, 주요우울장애, 불안장애 진단군에서는 모든 항목에서 유의미한 증상 호전 결과를 보였다. 결 론 : TF-CBT와 EMDR 치료를 받은 범죄피해자의 임상 증상 자기보고식 척도 점수는 유의미한 호전을 보였다. 더욱 효과적이고 효율적인 범죄피해자 치료지원을 위하여, 또한 대상자의 특성에 따라 적용 가능한 치료 프로토콜을 마련하기 위해 대상 센터와 대상 자를 확대한 추가적인 연구가 필요하다.

영문 초록

Objectives : The aim of this study was to investigate the preliminary effectiveness of the combination of trauma-focused cognitive behavioral therapy (TF-CBT) and eye movement desensitization and reprocessing (EMDR) in victims of crime in Korea. Methods : A non-controlled pilot study with 19 patients at Incheon Smile Center for Victims of Crime from 2014 to 2018 was conducted. Korean version of the Posttraumatic Diagnosis Scale (PDS), Korean Version of the Impact of Event Scale-Revised (IES-R), Korean-Beck Depression Inventory-II (BDI-II), Korean-Beck Anxiety Inventory (BAI) were measured and analyzed at baseline and at the end of treatment. Results : PDS, IES-R, BDI-II, BAI scores were improved after the combination therapy of TF-CBT and EMDR. The posttraumatic stress disorder (PTSD) diagnosis group showed the significant improvement in all scales except for avoidance items in IES-R, and the improvement in all items in the adjustment disorder, major depressive disorder, and anxiety disorder groups. Conclusion : The combination therapy of TF-CBT and EMDR has been shown to improve self-report scale scores for clinical symptoms in victims of crime.
