자살 시도자의 경향과 변화 추이

국문 초록

목 적 : 이 연구의 목적은 강원도 내의 자살 시도자의 사회 인구학적 요인과 자살 위험요소 및 자살 시도자의 심각도, 진정성 그리고 자살 시도 이후의 정신과적 치료력에 관한 경향과 변화 추이를 보고자 함이다. 방 법 : 2004년 8월 1일에서 2005년 7월 31일(2005년 군, 56명)까지 1년간과 2010년 5월1일에서 2011년 4월 30일까지(2010년 군, 58명) 1년간 춘천 성심병원 응급실에 자살 시도 후 내원한 환자들에서 정신과 협진이 이루어졌던 56명, 58명의 환자를 연구대상으로 하였다. 정신과 의사에 의해 환자와 보호자를 대상으로 30분 이상의 구조적인 면담이 이루어졌다. Hamilton Depression Rating Scale(HAM-D)를 이용하여 우울증을 평가하였다. 자살 시도의 심각도를 평가하기 위한 척도로 위험 구조 평가 체계(RRRS : RiskRescue Rating system)를 사용하였다. 자살 시도자와의 면담과 전반적인 인상을 통해 자살 시도의 진정성을 3점 척도로 평가하였다. 자살 시도자의 추후 정신과적 치료력을 평가하기 위해 각 환자들의 응급실 기록, 입원시 기록, 협진 기록 및 추후 외래 의무기록을 후향적으로 검토하여 자료를 수집하여 평가 하였다. 결 과 : 2005년 군과 2010년 군을 비교하면, 갈수록 자살 시도의 심각도가 높아지고 있는 추세이며, 자살 시도의 진정성이 높은 것으로 나타났다. 2005년 군과 2010년 군에서 자살 시도 이후의 정신과적 치료에 관하여 살펴보았을 때 정신과적 치료를 받는 숫자는 시대와 상관없이 낮은 비율을 차지한다. 결 론 : 본 연구를 통해 시간에 따라 자살 시도의 의학적 치명도가 높은 방법을 택하고 있는 것을 알 수 있다. 강원도 지역의 인구구조적 측면에서 노인 인구가 많고 알코올 문제를 가진 인구가 많다는 점을 보았을 때 지역 특성상 자살 위험에 취약한 사람들이 많다는 것을 알 수 있다. 지역 사회에서 노인 인구와 음주 문제에 대해 관심을 가지고 이에 대한 적극적인 개입이 필요함을 확인하는데 의의가 있다.

영문 초록

Objectives : The objective of this research is to examine the demographic factors of suicide attempters in the Gangwon-do region, suicide risk factors, the suicide attempter’s seriousness, sincerity, and the trends and changes in psychiatric healing process after suicide attempts. Methods : The subjects were the patients who visited the emergency room of Chuncheon The subjects were the patients who visited the emergency room of Chuncheon Sacred Heart Hospital after suicide attempts and received psychiatric treatments, and these patients were divided into the 2005 Group composed of 56 patients who visited the hospital for one year from August 1, 2004 to July 31, 2005, and the 2010 Group composed of 58 patients who visited the hospital for one year from May 1, 2010 to April 30, 2011. The structured interviews were conducted by psychiatrists for more than 30 minutes on each of those patients and caretakers. The patients were interviewed using Hamilton Depression Rating Scale (HAM-D), Risk-Rescue Rating system (RRRS). Through the interviews with the suicide attempters and overall impressions, the sincerity of the suicide attempts was evaluated on a 3-point scale. In order to evaluate the follow-up psychiatric healing process, each patient’s emergency room records, hospitalization records, medical records, and follow-up outpatient records were examined retrospectively and the data thus collected were evaluated. Results : The results of comparing the 2005 Group with the 2010 Group showed that both seriousness and sincerity of suicide attempts were getting increasingly higher as years went by. The results of examining the psychiatric treatments after suicide attempts in the 2005 Group and the 2010 Group showed that the number of those who received psychiatric treatments was low regardless of the times. Conclusion : Through this research, it can be seen that the methods of suicide attempts with higher lethality have been used as times go by. From the fact that the Gangwon-do region has many aging people and people with alcohol problems in the demographic aspect, it can be seen that it has many people vulnerable to suicidal risks due to regional characteristics. This research is meaningful in confirming that the local community should pay attention to the aging population and people with alcohol problems and that its active intervention is needed.
