인터넷에 관한 망상을 보인 정신증 환자 증례

국문 초록

연구목적： 정신증 환자의 망상 내용은 사회문화적 특성 및 시대적 배경을 반영한다. 저자들은 국내에서 널리 보급된 인터넷이 정신증 환자의 망상 체계에 어떻게 관여하였는지를, 증례를 통해 보고하고자 하였다. 방 법： 2001년 3월부터 2002년 10월 사이에 조선대학교병원 정신과에 입원한 정신증 환자들 가운데, 인터넷과 관련된 망상을 보였던 환자 3명의 병력을 기술하였다. 증 례： 2명은 여자이고 20대 후반이었으며, 1명은 남자이고 10대 후반이었다. 정신증의 발병은 첫 발병이 2명, 첫번째 재발이 1명이 었다. 환자들은 모두 주요 증상으로써 인터넷에 관한 망상을 보였고, 그 증상때문에 상당한 주관적 어려움을 겪었다. 치료를 통해 그 증상이 호전되기는 하였지만, 증상에 대한 통찰은 갖지 못했다. 결 론： 본 증례는 국내 정신증 환자에서도‘인터넷에 관한 망상’이라는 새로운 증상이 출현하였음을 의미한다. 인터넷 사용이 증가함에 따라 이 증상은 더욱 빈번해질 것으로 보이며, 앞으로 정신증을 진단하는데 중요한 증상이 될 것으로 예상된다.

영문 초록

Objective：Content of delusions in psychotic patients reflects socio-cultural features and phases of the times. This case report aimed to give an account of how the internet, widely used in Korea, took part in the delusional systems in patients with psychoses. Methods：Among the patients with psychoses, hospitalized in Department of Psychiatry of the Chosun University Hospital from March 2001 to October 2002, case histories of the patients with delusions about the internet were described. Case Reports：Of the three psychotic patients with delusions about the internet, two patients were female and in their late twenties, one patient was male and in his late teens. In terms of psychotic episodes, two were in their first episodes and one was in the second. The internet took part in the formation and content of delusions in the patients, and the symptoms gave considerable distress to them. Although those symptoms were improved with treatment, the patients failed to get insight on those. Conclusions：This case reports suggested that the new symptoms on ‘delusions about internet’ has appeared in Korean patients with psychoses as well. With the increased use of the internet, it is estimated that these symptoms may be more frequently reported, and may become important features in diagnosing psychoses.
