국문 초록

1995년 정신보건법의 제정 이후 정신보건체계가 기존의 병원 중심에서 지역사회 중심으로, 치료 위주에서 사례관리와 재활 위주로 전환되고 있다. 지역사회 정신보건사업(이하 지역정신보건사업이라 함)이란 지역사회에서 행해지는 모든 정신보건관련 활동을 의미한다. 이러한 지역정신보건사업을 원활하게 수행하는 데는 조직체계가 필요하다. 지속적이고 포괄적인 정신보건 서비스를 지역사회에 제공하기 위해서 정신보건센터는 가장 유용하며 선호되고 있는 모델이다. 정신보건센터는 정신보건사업의 조직화, 정신건강증진 및 정신질환예방사업, 그리고 정신질환자관리, 사업홍보 및 주민교육, 사업인력 교육 및 훈련 등의 역할을 수행한다. 경기도 지역정신보건사업에서 대부분의 정신보건센터는 대학병원 정신과나 전문정신병원에 위탁하여 운영되고 있다. 이와 달리 하남시는 보건소에서 직접 운영하는 정신보건사업으로서 경기도에서 유일하게 정신보건센터을 설립하였다. 개원한 정신과전문의가 자문의로서 주1회 참여하면서 정신질환자의 약물치료와 집단정신치료를 비롯한 전반적인 지역정신보건사업에 대한 자문을 담당하고 있다. 본 보건소중심형 정신보건센터 모델은 정신과전문의의 개입을 최소화하면서 주간보호을 중심으로 하여 정신보건사업을 통합하고 관리해 나갈 수 있는 모델이다. 본 논문에서는 우선 보건소형 정신보건센터 모델(중소도시 모델)의 전체적인 특징 및 체계와 조직을 소개하고, 이 모델의 정착과 정에서 나타난 여러 문제점과 특징을 살펴보며, 나아가 중소도시의 정신보건사업에 효과적으로 적용될 수 있는 모델로서 일반화할 수 있는 방법을 모색하고자 하는데 그 목적을 두었다.

영문 초록

The mental health system has been changed from hospital-orientd to community-oriented, from treatment to case management and rehabilitation after the promulgation of mental health law in 1995. Community mental health service performs all the activities within the community. The structure and system are inevitably needed to perform community mental health service smoothly. Community mental health center is the best usable and favourable model for the supply of continuing and comprehensive community mental health service. Community mental health center performs the roles which are composed of the organization of community mental health service, mental health promotion, the prevention of mental illness, the management of the chronically ill, public relation & public education, and the education and the training of personnels. Most of the community mental health centers in Kyonggi Province are deligated by the psychiatric department of university hospital or private mental hospital. On the other hand, Hanam health center established the mental health center from the starting point and has been managing it directly. The psychiatrist who has his private clinic in same area participates weekly as a consultant with the community mental health center. And he has a role of the medication for patients, group psychotherapy and consultation for community mental health service on the whole. This is the community mental health model led by health center that is operated by the least participation of the psychiatrist, and the psychiatrist is able to integrate and manage the community mental health service in the center of day care. The purposes of this study are as follows；introducing the general characteristics and the general system & structure； examining several problems and phenomena exposed in the process of settlement of this model；and brooding the possibility for generalization as a model applicable effectively in community mental health service of small or middle city.
