임신 중 약물남용이 자녀의 행동발달에 미치는 영향

국문 초록

본 연구는 임신 중 어머니의 음주, 흡연, 마약 등의 약물남용이 자녀가 3세가 되었을 때의 행동발달에 미치는 장기적 영향을 분석하였다. 특히, 그러한 영향이 출산 후 어머니의 정신건강 및 양육 환경에 의해 매개되는지 분석하였다. 미국 내 전국적 대표성을 가진 취약가정 아동에 대한 복지패널자료를 이용한 구조모형 분석결과, 출산 후 어머니의 부정적 양육태도가 임신기 약물남용이 아동에 미치는 영향을 부분적으로 매개하는 것으로 나타났다. 임신기 어머니의 약물남용은 출산 이후 모-자간 상호작용에 부정적 영향을 미칠 뿐만 아니라 자녀에 대한 체벌사용을 야기함으로써 자녀의 내향적·외향적 문제행동을 증가시키는 것으로 나타났다. 임신 중 약물남용이 자녀의 내향적·외향적 문제행동에 미치는 직접효과 또한 유의하게 나타났다. 연구결과를 바탕으로 임신기 약물남용에 대한 적극적 예방 및 치료의 필요성, 그리고 어머니의 부정적 양육태도에 대한 실천적 개입에 대한 필요성을 제시하였다.

영문 초록

Using three waves of a large longitudinal sample, this study examined the potential mechanisms through which substance use during pregnancy affects behavioral outcomes of children at age 3 via postnatal maternal depression and parenting at age 1. Findings from structural equation modeling analysis showed partial support for the hypothesized mediation effects. The maternal use of substances during pregnancy had a negative effect on mother-child interactions and tended to lead to maternal use of spanking at age 1, which, in turn, increased children's externalizing and internalizing behavior problems at age 3. The direct effects of substance use during pregnancy on both types of behavior problems of children were still significant after controlling mediators. These results provide evidence for the long-term detrimental effects of substance use during pregnancy on children's behavioral outcomes, and underscore the need for interventions addressing parenting difficulties among substance-abusing mothers.
