비판적 의료인문학과 거꾸로 의학교육

국문 초록

의학과 의료에 대한 대중의 이미지는 언제나 이중적이다. 냉철한 과학과 따뜻한 인간애가 공존해야만 하는 영역이기 때문이다. 과학이 의학의 전면에 등장한 이후 의학은 전대미문의 황금기에 접어들었지만, 다양한 사회문화적·윤리적 스캔들에 휩싸이기도 했다. 의료인문학은 이런 문제에 대한 해결책으로 등장했다. 하지만 과학이 주도하는 의료 현장에서 인문학은 과학적 의학의 치부를 가리는 역할에 머물렀다는 것이 ‘비판적’ 의료인문학자들의 주장이다. 그들은 과학과 의사가 중심이었던 의학을 인문학과 환자가 중심인 체계로 전면개편해야 한다고 주장한다. 질병을 일으키는 생물학적 사건도 질병을 앓는 환자의 경험을 중심으로 재구성해야 한다는 것이다. 이러한 흐름을 서사적 전환이라 한다. 서사적 전환의 흐름은 의학교육에도 영향을 미쳐, 교수가 전달하는 지식이 아니라 지식과 술기를 익히며 스스로 변해가는 학생의 경험이 중심이 되어야 한다는 자각에 이르게 된다. 거꾸로 교실은 이러한 반성에 입각한 전환(transformative)교육의 실천 방식이다. 추상적 지식이 아닌 구체적 현실 속 문제를 함께 풀어가는 과정에서 동료 그리고 환자와 함께 스스로의 정체성을 만들어 가는 존재의 미학이다. 존재의 미학에 근거한 의학교육에서는 화가가 대상 사물 속에서 특별한 현상을 찾아내어 표현하듯이 환자의 경험 속에서 의학적으로 의미 있는 징후를 찾아내는 감식안 그리고 환자가 경험하는 고통을 목격하고 공감하는 감수성을 키워야 한다.

영문 초록

The public image of medicine and medical practice have always been two-faced. Medicine is the area where cold science and warm humanistic services must coexist. Since the introduction of science into medicine it has achieved unprecedented success, but also has been involved in various socio-cultural and ethical scandals. The medical humanities have emerged as solutions to these problems. However, critical medical humanities advocates the claim that the humanities have only served as a cover-up for the shameful disgrace of the scandals. They argue that science and physician-centered medicine should be fully reorganized into a system revolving around the humanities and patients. The biological event that causes the disease should be reconstructed around the experience of the patient with the disease. This trend is called the narrative turn. The streams of the narrative turn also have impact on medical education. Medical educators are now advised to focus on the students’ experience during their courses of education. The experiences, rather than the knowledges and skills, transform the students’ selves. The flipped classroom is a form of this transformative education based on this patient-and-student-centeredness. It is the aesthetics of being that creates self-identity with peers and patients in the process of solving problems in concrete reality rather than abstract knowledge. In medical education based on aesthetics of being, learners should develop sensibility that detects medically meaningful characteristics among messy experiences of patients. They are like painters who catch meaningful representative characteristics among messy objects. Learners should also develop sensitivity witnessing and empathizing with the suffering of patients.
