국문 초록

의학의 과제는 사람의 생명을 살리는 것이다. 하지만 그 과제를 수행하는 방식과 그것을 떠받치는 사유의 양식은 시대와 문화에 따라 큰 차이를 보인다. 고대에는 실천적 지혜(phronesis)가 강조되었지만 근대 이후에는 추상화된 개념과 언어를 중심으로 한 사유(cogito)가 중심이 된다. 코기토의 의학은 엄청난 성과를 이루어냈지만 질병 그 자체에 초점을 맞추게 되면서 질병을 앓는 사람의 일상적 삶과는 유리된 지식체계로 발전했다는 비판에 직면해 있기도 하다. 이 논문은 의학 본연의 과제인 ‘생명살림’에 초점을 맞춘 새로운 사유 양식을 제안하기 위한 것이다. 그것은 모든 생명에 내재된 본질적 지향인 존재보전의 노력 즉 코나투스(conatus)이다. 먼저 코나투스의 개념을 제시한 스피노자의 내재적 자연관을 살펴본다. 그리고 스피노자의 자연 특히 코나투스와 의학 본연의 과제인 생명살림의 긴밀한 관련성을 논증할 것이다. 현대의학의 철학적 토대이며 코기토 의학의 출발점이기도 한 심신이원론의 대안으로 제시된 심신평행론이 생명살림이라는 의학의 과제를 수행하고 질병을 경험하는 환자의 삶을 설명하고 개선하는 데 적합함을 주장할 것이다. 3장에서는 진화, 면역, 인지 등의 분야에서 쏟아지고 있는 현대과학의 연구성과들을 스피노자의 자연 특히 코나투스의 개념으로 포괄해서 설명할 수 있음을 보여줄 것이다. 4장에서는 이런 과학적 성과들을 동아시아의 전통사상 및 스피노자의 자연과 묶어 하나의 흐름으로 읽어낼 것이다. 그리하여 형신감응의 우주론, 관계의 존재론, 감성 인식론, 기쁨의 윤리학을 제안한다. 첨단과학과 통접(統接)된 스피노자의 자연은 의학 본연의 과제인 생명살림을 수행하는 데 있어 아주 유용한 방향타가 될 것이다. 이것은 코기토를 중심으로 한 생물의학을 보완하는데 그치지 않고 점차 그것을 대체해 갈 수 있을 것으로 기대된다. 스스로의 몸을 살아내는 동시에 다른 몸을 살리는 몸살이와 몸살림의 의학 또는 코나투스의 건강학이다.

영문 초록

The ultimate task of medicine is to save and enliven the human life. But the ways of performing the task and the modes of thought holding up the ways are different according to the age and culture. In ancient times, practical wisdom (phronesis) was emphasized, but after approximately the 17th century, cogito utilizing abstract concepts and languages came to the center stage. The medicine stemming from cogito has achieved tremendous success. It also has been criticized being developed into knowledge of diseases rather than the people’s lives suffering from them. The purpose of this paper is to propose a new style of thinking that starts from the ultimate task of medidine, i.e., saving and enlivening life. I propose conatus, which is the force in every animate creature toward the preservation of its existence, as the new starting point of medicine. First, I examine Spinoza’s intrinsic view of nature from which the concept of conatus arises. Then I discuss the close relationship between Spinoza’s conatus and the medicine’s ultimate task. I argue that the mind-body parallelism proposed as the alternative to the mind/body dualism is appropriate for accomplishing medicine’s task and also for explaining and improving the patients’ life experiences. In section 3, I will show that conatus can be the covering concept conjoining the research results of modern sciences pouring out in the field of evolutionary biology, immunology, and cognitive sciences. In the fourth section, I will read the research results, traditional East Asian thought, and Spinoza’s nature into one big coherent flow. Then I explicate it into cosmology of mutual feeling and response between Form and Spirit(形神感應), ontology of relations, affective epistemology and ethics of joy. Spinoza’s Nature conjoined with advanced modern sciences will be a useful rudder with which to carry out the medicine’s ultimate task. Conatus-based medicine will not only complement cogito-based biomedicine but also is expected to encroach the basis of it. This will lead to the body-living and body-enlivening conatus medicine.
