푸르처 유사 망막병증이 첫 증상으로 나타난 전신홍반루푸스

국문 초록

목적: 전신홍반루푸스(systemic lupus erythematosus)의 첫 징후로 푸르처 유사 망막병증이 나타난 임상 증례를 경험하여 보고하고자 한다. 증례요약: 평소 건강하던 44세 여자 환자가 3일 전부터 발생한 시력저하를 주소로 내원하였다. 안저검사에서 양안 후극부의 다발성면화반 및 망막출혈이 관찰되었다. 형광안저혈관조영에서 양안 후극부의 비관류 및 혈관 누출이 관찰되었으며, 빛간섭단층촬영에서는양안 망막신경섬유층 및 신경절세포층의 증가된 고반사, 망막내액 및 망막하액이 관찰되었다. 류마티스내과와 협진하여 혈액응고이상을 포함한 혈액검사를 시행하였으며, 고역가의 anti-dsDNA 항체, anti-Sm 항체 및 임상양상을 토대로 전신홍반루푸스로 진단되었다. 전신 스테로이드 및 하이드록시클로로퀸 약제 투여 한 달 후 빛간섭단층촬영에서 양안의 망막내액 및 망막하액은 호전되었으나 교정시력은 호전을 보이지 않았다. 결론: 기저질환이 없는 환자에서 다수의 면화반 및 망막출혈은 푸르처 유사 망막병증일 수 있으며 전신홍반루푸스와 같은 자가면역질환으로 첫 발현할 수 있음을 고려해야 하겠다.

영문 초록

Purpose: To report a case of purtscher-like retinopathy as the primary manifestation in patient with systemic lupus erythematosus (SLE). Case summary: A 44-year-old healthy female presented with decreased visual acuity in both eyes. On fundus examination, multiple cotton wool spots and retinal hemorrhages on the posterior pole were observed. On fluorescein angiography, nonperfusion and vascular leakage were observed at the posterior pole of both eyes. On optical coherence tomography, there were increased hyperreflectivity in the retinal nerve fiber layer and ganglion cell layer, intraretinal fluid, and subretinal fluid. On serological tests including thrombophilia examination, anti-Ro antibodies, anti-dsDNA antibodies, anti-Sm antibodies were positive. She was diagnosed with SLE. After 1 month of systemic steroid and hydroxychloroquine treatment, the intraretinal fluid and subretinal fluid resolved, but visual acuity was not recovered. Conclusions: In patients without underlying disease, it should be considered that multiple cotton wool spots and retinal hemorrhages may be Purtshcer-like retinopathy and may appear as a primary manifestation of an autoimmune disease such as SLE.
