안구돌출로 나타난 기무라병

국문 초록

목적: 기무라병은 드문 만성 염증성 질환으로, 주로 두경부 부위의 무통성 피하 종괴로 나타나며 드물게 눈꺼풀이나 안와에도 발생한다. 저자들은 외안근의 비대와 안구돌출로 내원한 환자에서 기무라병을 진단하여 보고하고자 한다. 증례요약: 2년 전부터 서서히 진행하는 우안 안구돌출로 16세 남자가 내원하였다. 우안 위눈꺼풀부종과 안구돌출이 있었고, 안와 자기공명영상에서 우측 외안근이 모두 비대해져 있었다. 과거력 상 2년 전부터 좌측 팔 안쪽에 다발성 무통성 종괴가 있었고, 자기공명영상에서 좌측 팔꿈치 상완 동맥 주변 림프절들 크기가 커져 있음을 확인하여 이에 대한 절개 생검을 시행하였다. 조직학적 검사상림프절의 여포성 증식 소견, 곁피질에서 호산구성 농양 형성을 동반한 호산구의 증가 소견 및 관절낭 외 호산구 및 형질세포 침윤소견을 보여 기무라병으로 확진하였다. 경구 스테로이드 및 싸이클로스포린 복용 1년 후 안구돌출과 외안근 비대가 호전된 것을 확인하였다. 결론: 외안근 비대로 인한 단안의 안구돌출을 보였던 환자에서 진단한 기무라병을 보고하는 바이다.

영문 초록

Purpose: Kimura disease is a rare chronic inflammatory disease that usually presents with painless subcutaneous nodules at the head and neck area, but rarely occurs around the eyes. We report unique case of Kimura disease involving the orbit and arm, which has not been reported in Korea. Case summary: A 16-year-old boy presented with exophthalmos and eyelid swelling of right eye over the last 2 years. Orbital magnetic resonance imaging (MRI) showed hypertrophy of extraocular muscles of right eye. He also complained of multiple painless masses of the left forearm that persisted during the same period. The MRI showed hypertrophy of extraocular muscles of right eye and multiple lymphadenopathy along the brachial artery of the left elbow. Excisional biopsy of the left arm mass and lymph nodes was performed and the lesions were diagnosed as Kimura disease involving the dermal nodule and lymph nodes. The patient was managed with oral corticosteroid and cyclosporine and showed remarkable improvement of proptosis and hypertrophy of extraocular muscles. Conclusions: We report a rare case of Kimura disease causing hypertrophy of extraocular muscles which was successfully treated with immunosuppressive agents.
