잠재성장모형을 이용한 의료급여 수급자의 건강과 의료이용의 변화궤적에 관한 종단 연구

국문 초록

이 연구는 의료급여 수급자들의 주관적 건강상태와 미충족의료, 외래의료이용의 변화궤적과 종단적 상호관계, 그리고 변화궤적에 영향을 미치는 요인을 탐색하는 것을 목표로 하였다. 이를 위해 한국의료패널조사의 2011년부터 2018년까지 8개년 동안 의료급여 수급을 지속한 168명을 대상으로잠재성장모형을 활용한 분석을 수행하였다. 분석결과, 시간이 흐를수록 주관적 건강상태는 나빠지고미충족의료 경험률은 높아지며 외래이용횟수는 증가하는 것으로 나타났다. 초기 주관적 건강상태가나쁜 이들에서 미충족의료 경험률이 높았고 외래이용도 많았다. 미충족의료와 외래이용횟수는 초기값과 변화율 모두 상호 연관성이 존재하지 않은 것으로 나타났다. 초기 주관적 건강상태가 양호한 이들에게서 의료이용이 더 빠르게 증가했고, 초기 미충족의료를 경험한 이들에게서 주관적 건강상태의 악화가 더 빠르게 진행되는 것으로 나타났다. 수급지속기간이 길수록 초기 주관적 건강상태가 더 양호했으며 외래이용증가 속도가 상대적으로 느린 것으로 나타났다. 이 연구는 의료급여 수급자의 건강동태에 대한 최초의 종단적 연구라는 점에서 의의를 지니며, 이 연구결과가 향후 의료급여 수급자의건강향상을 위한 정책적 대안을 모색하는 데 근거로 활용될 수 있길 기대한다.

영문 초록

This study aimed to explore the trajectories of the change and the longitudinal correlations in self-rated health(SRH), unmet healthcare needs(UN) and outpatient service uses(OU), and the factors affecting these trajectories in medical aids(MA) recipients. For this, an analysis was conducted applying the Latent Growth Model for the 168 people who continued to receive MA from 2011 to 2018 in the Korea Health Panel. According to the analysis, the SRH declined, and the rate of experience of UN and the number of OU increased over time. Those with poor initial SRH had a higher rate of experience of UN and a more number of OU. The initial value and the rate of change between the experience of UN and the number of OU were found to have no statistical correlation. The number of OU increased faster in those with good initial SRH, and the decline of SRH was found to be faster in those who experienced initial UN. The longer the duration of receiving MA is, the better the initial SRH and the slower the rate of increase in the number of OU. This study is significant in that it is the first longitudinal study of health dynamics of MA recipients. We expect this result can be used for seeking policy alternatives to improve health of MA recipients
