중심오목부근 모세혈관확장증에서 광역학치료(PDT)와 Bevacizumab 주입술 2예

국문 초록

목적: 중심오목부근 모세혈관확장증 1A군으로 진단된 환자 2명에서 bevacizumab 유리체강내 주입술(IVBI)과 광역학치료(PDT) 병합요법의 임상경험 2예를 보고하고자 한다. 대상과 방법: 중심오목부근 모세혈관확장증 1A군으로 진단된 54세 남자환자에서 2회의 IVBI 및 1회의 PDT가 시행되었다. 중심오목부근 모세혈관 확장증 1A형으로 진단된 67세 여자환자에서는 1회의 PDT 및 3회의 IVBI가 시행되었다. 첫번째 환자의 첫 내원 시 교정시력 0.22 (LogMAR), 빛간섭단층촬영상 중심와두께는 367μm로 측정되었다. 결과: 치료 후 최대교정시력은 0.1, 중심와두께는 249μm로 호전되었다. 두번째 환자의 첫 내원 시 교정시력은 1.0, 중심와 두께는 345μm로 측정되었다. 치료 후 최대교정시력은 0.3, 중심와 두께는 197μm로 호전되었다. 두 환자 모두 형광 안저 촬영상 누출성 병변의 크기가 확연히 줄어들었으며, 치료로 인한 합병증은 관찰되지 않았다. 결론: 중심오목부근 모세혈관 확장증 1A군에 있어 IVBI와 PDT 병합요법으로 시력이 호전되고 병변이 안정화된 경험을 보고하는 바이다.

영문 초록

Purpose: To report two patients who were treated by photodynamic therapy and intravitreous bevacizumab injection in group 1a of Idiopathic Macular Telangiectasis (IMT). Methods: 54-year-old man who had been diagnosed as group 1a IMT in his left eye was treated by intravitreal bevacizumab injection twice and then photodynamic therapy in case 1. 67-year-old woman with group 1a IMT in her right eye was treated by photodynamic therapy (PDT) once and then intravitreal bevacizumab injection three times in case 2. PDT was performed for vascular telangiectasia area. Results: Visual acuity improved and optical coherence tomography showed decrease of macular edema in both cases after photodynamic therapy and intravitreal bevacizumab injection. Fluorescein leakage decreased, as well. Conclusions: We report two rare cases of group 1a of idiopathic macular telangiectasis. Intravitreal bevacizumab injection and PDT in patients with group 1a of idiopathic macular telangiectasis can result in a rapid decrease of macular edema, which will then lead to rapid visual recovery. We recommend that intraviteal bevacizumab injection be used in combination with PDT for the treatment of type 1a of idiopathic macular telangiectasis.
