성년기에 처음 발생한 양안 봄철 각결막염 1예

국문 초록

목적: 봄철 각결막염은 대개 10세 이전에 발병하여 사춘기에 대부분 없어지는 질환으로 알려져 있다. 저자들은 아토피 피부염을 가진 남자 환자에서 25세에 처음 발병하여 심한 지속성 각막 상피 결손 후 생긴 shield ulcer를 동반한 봄철 각결막염 1예를 보고하고자 한다. 대상과 방법: 25세 남자환자는 양안 시력 저하와 통증을 주소로 내원하였다. 아토피의 병력이 있었고 봄철각결막염이나 다른 결막염의 병력은 최근까지 없었다. 최대교정시력은 양안 0.06이었고 세극등 검사상 상안검결막에 거대유두, 각막 상피 결손, 주변부 신생혈관이 보였고 전방 내 염증소견은 보이지 않았으며 소양증과 피부염이 동반되어 있었다. 혈액 검사를 시행하고 자가 혈청 및 스테로이드 점안액을 투여하고 치료용 콘택트렌즈로 치료를 시작하였다. 결과: 혈액학적 검사상 혈청 IgE가 증가했고 호산구증다증이 있었다. 비만세포안정제를 추가로 점안하고 사이클로스포린을 경구 투여하여 양안 시력이 0.3로 호전되었다. 결론: 봄철 각결막염의 발병 시기는 주로 8세부터 12세 사이에 발병하며 25세 이후에는 발병이 드문 것으로 알려져 있으나 청장년기에 처음 발병한 봄철 각결막염 1예를 경험하였기에 문헌 고찰과 함께 보고한다.

영문 초록

Purpose: Vernal keratoconjuctivitis is known to have the onset before the age of 10 and subside after puberty. Authors report a case of a adult onset vernal keratoconjuctivitis accompanied with shield ulcer in 25 years old patient. Methods: 25 year old male was admitted complaining deterioration of vision and pain. Best corrected vision was both 0.06. History of atopic dermatitis was noted but no prior vernal keratoconjuctivitis. Slit lamp examination revealed superior palpebral giant papillae, corneal epithelial defect, neovascularization and no anterior chamber inflammatory signs. Hematologic examination was performed. Topical auto-serum and steroid was given and Therapeutic contact lens were inserted. Results: Hematologic examination revealed increased serum IgE and eosinophilia. Mast cell stabilizers were given in addition and cyclosporine was orally given. Both vision improved to 0.3. Conclusions: Vernal keratoconjunctivitis is a disease affecting youth in their first decade of life but rarely appearing in patients older than 25 years. We report a case of first onset vernal keratoconjunctivitis in adolescent period with the review of literature.
