당뇨병환자에서 유리체절제술 후 다초점 인공수정체의 지연성 혼탁

국문 초록

목적: 당뇨병환자에서 유리체절제술 후 AT LISA 인공수정체의 지연성 혼탁을 경험하여 보고하고자 한다. 증례요약: 61세 남자 환자가 좌안 흐림을 주소로 내원하였다. 그는 6년 전 양안 수정체초음파유화술과 인공수정체삽입술을 받았고, 3년 전에 좌안의 열공망막박리로 SF6 가스 안내 주입술과 유리체절제술을 받았다. 환자는 기저질환으로 당뇨병, 고혈압 및 심장 관상동맥질환이 있었다. 좌안 세극등현미경검사상 인공수정체 표면 중앙에 둥글고 흰색의 과립성 침착이 관찰되었고, 3시에서 6시 방향에서 섬모체소대 파열이 관찰되었다. 우안의 인공수정체는 깨끗하였다. 교정시력은 우안 0.9, 좌안 0.5였다. 좌안의 인공수정체는 수정체낭과 함께 제거하였고 광학현미경, 조직화학 분석, 주사전자현미경 분석을 시행하였다. 결론: 본 연구는 다초점 인공수정체 AT LISA 809M에서 수술 후 지연성 인공수정체 혼탁에 관한 최초의 보고로 기저질환이 있는 환자나 유리체절제술 시행 예정인 환자에서 인공수정체 혼탁의 발생 가능성을 적절하게 설명하여야 할 것이다.

영문 초록

Purpose: We report a case of late opacity of an AT LISA intraocular lens after vitrectomy in a diabetic patient. Case summary: A 61-year-old man presented with blurring in the left eye. He had undergone bilateral phacoemulsification and intraocular lens implantation 6 years ago, and vitrectomy with SF6 gas tamponade in the left eye due to rhegmatogenous retinal detachment 3 years ago. The patient was diagnosed with pre-existing diabetes, hypertension, and coronary heart disease. Examination of the left eye showed round and centrally located whitish fine granular deposits on the surface of the intraocular lens, and zonular rupture from the 3 to 6 o’clock position. The intraocular lens in the right eye was clear. The corrected vision was 0.9 in the right eye and 0.5 in the left eye. The intraocular lens was explanted from the left eye together with the capsular bag, and was examined under light microscopy, histochemical analysis, and with scanning electron microscopy equipped with an energy dispersive X-ray spectroscopy detector with light element capabilities. Conclusions: This study was the first to report late postoperative opacity in the multifocal intraocular lens, AT LISA 809M. The possibility of intraocular lens opacity in patients with underlying disease or in those undergoing vitrectomy should be explained prior to corrective procedures.
