건강보험 피부양자제도를 둘러싼 가족의 재구성

국문 초록

2011년 초 건강보험의 재정적자 문제가 사회적 이슈가 되면서, 일정 수준이상의 재산이나 소득이 있는 직장가입자 피부양자를 지역가입자로 전환시키고, 이들에게 보험료를 부과하는 보험료 부과체계 개선정책이 확대되기 시작하였다. 직장가입자 피부양자 중 사업소득 또는 과세가 되는 4,000만원을 초과하는 금융소득 소지자에 대한 지역가입자 전환의 강화 및 재산세 과세표준액 기준 9억원 이상의 재산 보유자의 경우 지역가입자로 전환 실시, 또는 종합소득 4,000만원 이상 소지자에 대한 지역가입자 전환 계획 등이 이러한 정책의 내용이다. 하지만, 이러한 보험료 부과체계 개선정책은 건강보험의 재정안정화를 위한 보험료 수입증대 방안이며, 재산이나 소득이 있는 직장가입자 피부양자를 둘러싼 사회적 논란에 대한 임시방편적 조치일 뿐 이다. 이러한 정책은 가입자를 직장가입자와 지역가입자로 구분하여 이들에게 서로 다른 기준으로 보험료를 부과하여 현행 보험료 부과체계를 재설계하자는 사회적 요구를 충족시키지 못하는 정책이다. 본 연구는 현행 건강보험 보험료 부과체계가 가지는 다양한 문제들 중 피부양자 인정 문제를 가족 보험과 개인보험의 차원에서 분석하여, 가족보험으로서 건강보험의 확립에 이론적으로 기여하고자 한다. 이를 위하여 가입자의 직역에 따라 직장가입자는 가족보험으로, 지역가입자는 개인보험으로 혼재된 현행 건강보험제도에서의 피부양자 문제를 분석한 후, 이를 해결하기 위한 다양한 이론적 연구들과 정책방안을 비판적으로 검토하고자 한다. 마지막으로 본 연구는 공적 의료보험제도에서 피부양자는 가입자와 동일한 가구를 구성하고, 가입자가 경제적으로 부양하는 자로 한정하는 원칙의 확립을 재강조하고, 가입자 직역간의 형평성문제를 해결하기 위하여 ‘기본보험료’를 도입하여 외형적으로는 건강보험을 개인보험으로 재편하지만, 실질적으로는 가족보험으로 재정립하는 방안을 제시하고자 한다.

영문 초록

Because of the social conflicts about the financial deficit in the National Health Insurance in the year of 2011, the government has tightened regulations on the ‘dependents’ in the NHI. The main contents were the compulsory change of the position, who has some income or wealth above a certain standard from the dependents of the insured-industrial workers to the insured-self-employees in rural/urban area. This policy for the refinement of the contribution levy system in the NHI is only a measure for increasing of contribution revenue or/and pacifying the social conflicts about the position of the dependents in the NHI. What is needed, is the redesign of the contribution levy system in the NHI, which leads to loosen or solve that social conflicts. This Study has explored the social conflicts about the position of the dependents in the NHI with the concept of the ‘family insurance’ and ‘individual insurance’. After a review of the last 9 analyses for the redesign or refinement of the contribution levy system and practical policies to loosen or solve that social conflicts, this study proposes the introduction of the basic contribution. With the basic contribution, the composition principle of the family members in the NHI would be unified in ‘individual insurance’ on external. But the NHI becomes ‘family insurance’ in reality, because diverse countermeasures would be made for the vulnerables at the same times. In the middle term, the basic contribution would provide positive possibilities for the unification of the separated position of the insured and the redesign of the contribution levy system in the NHI.
