국문 초록

본 연구는 보건 인력 중심으로 운영되고 있는 지역사회 생애말기 돌봄 서비스에 있어서 사회복지 개입의 필요성을 탐색해보고자 하였다. 이를 위해서 지역사회 돌봄, 생애말기 돌봄의 개념을 고찰한후, 지역사회 재가 암환자 및 생애 말기 클라이언트를 대상으로 호스피스완화케어를 제공하고 있는 실무자 및 관련 전문가를 대상으로 포커스그룹 인터뷰(FGI)를 실시하였다. 그 결과, 지역사회에 거주하는 생애말기 대상자가 가지는 욕구는 의료적 욕구를 넘어서는 복합적인 것이었으며, 보건인력 중심 의 현장 실무자들은 대상자에 대한 전인적 사정의 어려움, 사회복지정보 부족, 자원연계 업무 부담, 자원봉사자관리의 어려움, 호스피스에 대한 인식 부족으로 인한 어려움을 겪고 있었다. 또한, 사회복지 서비스 연계를 시도하지만 정보 공유 및 연계가 어려우며, 연계되더라도 사후 의사소통이 안 되고, 서비스의 질을 확보하기 어려움을 호소하였다. 현장 실무자들은 지역사회 생애말기 돌봄에서 보건과 복지는 겹쳐져 있는 것이라고 인식하고 있으며, 서비스의 효율성을 높이기 위해 보건-복지 영역 간 관계 맺기, 통합사례관리, 인력 교환의 필요성을 제시하였다.

영문 초록

This study tried to inquire he necessity of social work intervention in community-based end of life care run by health personnel. For this, we reviewed concept of community care and end of life care and then conducted focus group interview(FGI) against hands-on staffs and relevant experts who provide hospice palliative care to house staying cancer patients and client of end of life care in community. The result shows that the desire of end of life patients is complex beyond medical desire and that hands-on staffs primarily consisting of health personnel have difficulties due to whole people s circumstances, shortage of social welfare information, burden of work connected to resource, control of volunteers and shortage of personnel in hospice. Additionally, they say that sharing and connection of information is difficult in trying to connect social welfare service and even after successful connection, communication is not available and it is not easy to secure quality of service. Hands-on staffs recognize that health and welfare in end of life care of community is overlapped. This study has suggested that in order to improve efficiency of service, building relationship between health and welfare, control of unification examples and exchange of personnel is required.
