국문 초록

도시에는 환경과 서비스, 기회가 더 잘 갖춰진 이점이 있는 한편으로 빈곤, 실업, 소외, 불균등 발전에 따른 불리함도 있다. 도시건강은 세계적인 도시화 추세와 함께 도시의 문제와 건강이 주고받는 상호영향에 주목하는 분야이다. 최근에는 도시의 정책과 계획이 건강과 형평을 지향하도록 하고, 이를 추진하는 과정에서 부문 간의 협력을 지향하는 것이 도시건강의 주요 전략이다. 이러한 경향을 바탕으로 본 연구에서는 도시건강에 대한 최근 동향을 살펴보고, 국내 도시건강의 한 사례로 서울시 자 치구 건강도시에 대한 심층면접조사를 실시하였다. 근래 국제적으로는 도시건강에 대한 다학제적 접근이 기본 전제이자 과제이다. 연구주제로는 도시의 건조환경(built environment)과 신체활동, 식생활, 비만 간의 관계가 떠올랐다. 이러한 연구경향과 함께 도시건강 전략은 도시 여러 부문의 계획과 다양한 정책이 건강을 고려하도록 건강영향평가를 도입하거나 다부문 협력을 통해 건강지향적인 도시계획을 추진하는 방향으로 전개되고 있다. 대표적으로 세계보건기구의 건강도시 활동에는 건강영향평가와 도시건강계획이 포함되어 있다. 유럽의 건강도시는 건강의 형평성을 목표로 하여 건강정책 실현을 위한 리더십과 거버넌스에 중점을 두고 있다. 국내에서는 지난 10여 년간 건강도시의 도입과 전개가 현장과 연구의 주요 주제였다. 최근에는 활동적인 생활환경 조성이 국내 건강도시의 중심주제가 되어 도시계획과 건강도시의 연계 필요성이 부각되고 있다. 다만 서울시 자치구 건강도시 대상 심층면접조사에서는 예산 문제가 장애로 지적되었고, 건강증진과 건강도시 간의 개념 정리 필요성이 파악되었다. 따라서 도시건강을 발전시키기 위해서는 건강증진과 건강도시의 관계에 대한 개념과 인식을 재정리하고 도시건강에 대한 지속가능한 노력이 실현되도록 옹호활동이 필요하다. 그밖에 도시계획과 보건의 통합적 접근을 지원하고, 관련 분야들이 교류·협력할 수 있는 구체적인 계기를 마련해야 한다.

영문 초록

Cities have urban advantages associated with relatively better services and opportunities. Meanwhile, cities also have disadvantages induced by urban poverty, sprawl, and inequity. In this urbanizing world, the focus of urban health is on the interplay between urban issues and health. For urban health with an emphasis on health equity, integration of health in urban policies and plans is emphasized as well as the importance of intersectoral urban health approaches. In this context, this study aims to examine the current trends of urban health research and practice. As part of the study a qualitative interview was conducted on the situation of the Healthy Cities initiatives in districts in Seoul. Interdisciplinary approaces, and the association between built environment and physical activity, diet, obesity, and cardiovascular diseases are salient issues in current urban health research. In practice, instrumental health-oriented urban strategies include Health Impact Assessment, and integration of urban planning and public health. The Healthy Cities initiative has been a focal issue in research and practice in Korean cities in a past decade or so. Recent interest in Korean Healthy Cities is building active living environment and linking urban planning and the Healthy Cities. In practice, shortness of budget was a common challenge. The need for clarifying the relationship between the Healthy Cities and health promotion calls for attention. Urban health strategies should address: the creation of specific and practical opportunities for intersectoral, interdisciplinary collaboration; the clarification of the relationship between the Healthy Cities and health promotion; and the advocacy for sustaining efforts for urban health.
